Community Health Worker
South Central Public Health District has an exciting career opportunity with our Parents as Teachers home visiting team. We are seeking a full-time Community Health Worker to join a dynamic team of professionals who share a common vision of promoting public health in our communities. This position is located in our Heyburn office and will serve families who are primarily in Minidoka and Cassia counties.
This position, with supervision, will provide home visits to families enrolled in the Parents as Teachers (PAT) model. The visits will be provided in the home setting. Interventions and screenings are provided as part of evidence-based curriculums designed for use with this model. Services are provided to families with young children, age 0-5 years.
