Community Health Worker

South Central Public Health District has an exciting career opportunity with our Parents as Teachers home visiting team. We are seeking a full-time Community Health Worker to join a dynamic team of professionals who share a common vision of promoting public health in our communities. This position is located in our Heyburn office and will serve families who are primarily in Minidoka and Cassia counties.

This position, with supervision, will provide home visits to families enrolled in the Parents as Teachers (PAT) model. The visits will be provided in the home setting. Interventions and screenings are provided as part of evidence-based curriculums designed for use with this model. Services are provided to families with young children, age 0-5 years.

ESSENTIAL FUNCTIONS :

  • Conduct home visits for pregnant women, newborns, or high-risk individuals to monitor progress and assess needs.
  • Engage with families in their homes, regardless of home conditions, unless deemed unsafe.
  • Interact with parent / child on the floor, without accommodations.
  • Drive to / from visits without accommodations, unless unsafe to do so.
  • Perform all home visits in person, unless unsafe or requested otherwise by the family.
  • Follow the Parents as Teachers (PAT) curriculum and meet all PAT essential requirements.
  • Adhere to PAT and Maternal, Infant, Early Childhood Home Visiting (MIECHV) screening and documentation standards.
  • Document visits and related screeners in no more than 3 days.
  • Assist in planning and attending all Group Connections.
  • Participate in staff meetings and reflective supervision sessions.
  • Contribute to Continuous Quality Improvement (CQI) projects.
  • Complete home visit audits as directed.
  • Conduct visits during weekends or evenings as needed.
  • Represent the District at outreach events to build relationships and educate the community.
  • Respond promptly to team requests and communications.
  • Assist families in applying for social services (e.g., Medicaid, WIC) and provide cultural mediation for health services.
  • Advocate for community health needs with agencies and providers.
  • Promote SCPHD services and collaborate with community partners.

  • Experience providing care, treatment, and / or recommending services to varying cultural / socioeconomic backgrounds based on client needs
  • Some knowledge of child development
