Preferred Experience:

1. Working with people with Developmental Disability or in the Development Disability services system

2. Completing assessments or program eligibility decisions

3. Candidates with professional working proficiency in Spanish may be preferred.

Brief description - Conduct interview with individuals requesting Waiver services, complete Screening for Urgency of Need (SUN) questionnaires, and assign level of need based on results of the SUN.


NOTE REGARDING THE ADVERTISED PAY: 


This position participates in a Career Progression Group (CPG) and may be filled as a COMMUNITY SERVICE PROFESSIONAL  3  level at which the vacancy is filled will be determined by the qualifications of the candidate selected. This position provides the opportunity to advance within this job series. 

The actual starting salary depends on the education and experience of the selected applicant.

MINIMUM QUALIFICATIONS:
Four years of experience in providing health or developmentally disabled services, counseling, health care management, or social services; OR

Six years of full-time work experience plus one year of experience in providing health or developmentally disabled services, counseling, health care management, or social services; OR

A bachelor's degree plus one year of experience in providing health or developmentally disabled services, counseling, health care management, or social services; OR

An advanced degree in a social sciences field, nursing, public health; public, hospital, or health administration; speech-language pathology or communication sciences and disorders; physical, occupational, recreational, or music therapy.

EXPERIENCE SUBSTITUTION:
Every 30 semester hours earned from an accredited college or university will be credited as one year of experience towards the six years of full-time work experience in any field. The maximum substitution allowed is 120 semester hours which substitutes for a maximum of four years of experience in any field.
FUNCTION OF WORK:
To receive on-the-job training for entry level implementation, coordination, monitoring and evaluation of programs within offices specializing in services for those with developmental disabilities.
 
LEVEL OF WORK:
Entry.
 
SUPERVISION RECEIVED:
Close from a Community Services Supervisor, a higher-level professional or an administrator.
 
SUPERVISION EXERCISED:
None.
 
LOCATION OF WORK:
Department of Health and Hospitals (DHH) and Human Services Districts/Authorities statewide.
 
JOB DISTINCTIONS:
Differs from the Community Services Professional 2 by the presence of close supervision required in carrying out entry-level responsibilities.
EXAMPLES OF WORK:
EXAMPLES BELOW ARE A BRIEF SAMPLE OF COMMON DUTIES ASSOCIATED WITH THIS JOB TITLE. NOT ALL POSSIBLE TASKS ARE INCLUDED.
 
Assists clients in developing service plans including identification of goals and appropriate services and making recommendations for specific support needs.
 
Conducts assessments of clients utilizing standardized assessment tools.
 
Prepares appeal packages for submission to the Bureau of Appeals and represents agency at hearings. Attends appeal hearings to learn the processes and procedures.
 
Prepares reports, maintains records, and conducts correspondence relevant to areas of specialty.
 
Interviews and establishes working relationships with clients and families.
 
Serves as a liaison with a variety of local and state agencies, professional organizations, advocacy organizations, legal and court related entities, and private providers to develop, maintain, and enhance a viable service delivery system for individuals with developmental disabilities and their families.
 
Coordinates a portion of a statewide program element in a specific service area, under close supervision.
 
Provides supportive services for clients and assists in obtaining services from appropriate community resources based on the support plan.
 
ENTRY UNIT:
Assesses home environment, living situation, and abilities of individuals and families to determine the need for support service. Also determines if other agency referrals are necessary for crisis intervention.
 
Completes standardized assessments for determining eligibility of Developmentally Disabled state and federally/mandated programs and services.
 
Determines the current status of clients to evaluate if an immediate danger/risk is present.
 
Obtains medical, psychological, educational reports on clients; determines if information supports eligibility requirements as established by state law.
 
Prepares eligibility decisions.
 
FAMILY SUPPORT UNIT:
Implements the developed service plan and monitors compliance by providers, family members and others to ensure supports are delivered satisfactorily. 
 
Participates in setting case plan goals/objectives in family team conferences and presenting requests for funding as appropriate.
 
Monitors the flow of required documents for fiscal oversight of service plan.
 
Observe upper level staff performing on-site interviews with provider agency administrators; assure program compliance with contract expectations.
 
Coordinates exploration of alternate living arrangements for clients/families requesting such, as outlined in plan of services.
