Main Purpose of Job
The main purpose of this part-time contractual position is to conduct a comprehensive evaluation to identify public services available to assist frail, aged, or disabled adults and children to remain in the community, or in their least restrictive environment, with as much independence as possible. AERS is a Maryland program administered by the Talbot County Health Department.
Evaluations include the utilization of the State required STEPS and interRAI evaluation tools and are completed in the client's current place of residence, including all community residential settings, nursing facilities, and hospitals.
This position also provides case management through care plan development and implementation to assist the individual in obtaining the services recommended in the plan of care.
This position acts as a Nurse Monitor for clients receiving Community First Choice (CFC) services, Community Personal Assistance Services (CPAS), or are enrolled in the Community Options (CO) waiver programs. Duties include providing quality assurance and care plan monitoring for clients in the community who are Medicaid recipients receiving personal care services and who have a chronic illness, medical condition, or disability.
This position supports the mission of Talbot County Health Department, which is to promote and improve the health and safety of Talbot County residents, by helping Talbot County's frail, aged, or disabled adults and children to live in the safest and least restrictive environment possible and fostering their independence and personal welfare.
Minimum Qualifications
Education: A Bachelor’s degree in nursing from an accredited college or university.
Experience: None.

Benefits
Contractual employees who work for an agency covered under the State Employee and Retiree Health and Welfare Benefits Program, have a current employment contract and work 30 or more hours a week (or on average 130 hours per month) may be eligible for subsidized health benefits coverage for themselves and their dependents. As a contractual employee, you will be responsible for paying 25% of the premiums for your medical and prescription coverage, including any eligible dependents you have enrolled. The State of Maryland will subsidize the remaining 75% of the cost for these benefits. You can also elect to enroll in dental coverage, accidental death and dismemberment insurance, and life insurance, but will be responsible to pay the full premium for these benefits.
Leave may be granted to a contractual employee at a rate of one hour for every 30 hours worked, not to exceed 40 hours per calendar year.

- Candidates may substitute a Bachelor’s degree in a related field from an accredited college or university and one year of experience as a Registered Nurse for the required education.
- Candidates may substitute two years of experience as a Registered Nurse for the required education.
- Candidates may substitute U.S. Armed Forces military service experience as a commissioned officer in Nursing classification or Nursing or Health Care Specialist specialty codes in the Nursing field of work on a year-for-year basis for the required education.
