Why does the job exist?
The Healthcare Surveyor III/Intake Coordinator will:
Triage reports of abuse, neglect, exploitation, suspicious injuries, environmentally hazardous conditions and death for individuals on the developmental disability waiver, Mi Via Self-Directed waiver, the Supports waiver, and the Medically fragile waiver;
Be responsible for taking ANE reports through the hot line, monitoring the ANE on-line reporting system and checking daily for ANE faxes, and procuring Immediate Action and Safety Plans;
Obtain information to determine jurisdiction and authority to assign pursuant to state regulations and make screening decision.
How does it get done?
The Healthcare Surveyor II/Intake Specialist is responsible for:
Receiving calls through the ANE hot-line and receive on-line ANE reports via the APS on-line portal
Receiving information, creating a tip sheet containing the information reported and obtains an Immediate Action and Safety Plan
Making a screening decision
On-call working hours
Who are the customers?
The Individuals who received Medicaid waiver services such as the DD waiver, Mi Via waiver, Supports waiver, and the Medically Fragile waiver.
Ideal Candidate
The Individuals who received Medicaid waiver services such as the DD waiver, Mi Via waiver, Supports waiver, and the Medically Fragile waiver.
Minimum Qualification
Associates degree from an accredited college or university in Nursing, Architecture, Social Work, Sociology, Psychology, Rehabilitation, and/or Criminal Justice and two (2) years directly related experience in community based healthcare services and case management. Any combination of education from an accredited college or university in a related field and/or direct experience in this occupation totaling four (4) years may substitute for the required education and experience.
Employment Requirements
Must possess and maintain a valid New Mexico Driver's License. Must Possess and maintain a current Defensive driving Course Certificate from the State of New Mexico or must pass and receive Defensive Driving Course Certification as a condition of continued employment.
Working Conditions
Working knowledge of and extensive use of the telephone, Microsoft Word, Visual/Video Display terminal, basic computer knowledge, basic typing skills, professional customer service skills.
Must be available for On Call (after working hours/weekends).
