Why does the job exist?Incumbent will support the investigation, permanency and placement unit staff in gathering collateral information for investigations and contacting collateral supports, entering documentation in the case records and supporting investigative, placement and permanency staff while in the field when individual visits are of concern. This position will support caseworkers by making referrals for services for children and parents, completing documentation in FACTS for the workers, setting up appointments for caseworkers to visit children, parents and resource parents, completing payments in FACTS for resource parents and providers, supervise community visits, help search for relatives, tracking down records, scheduling and tracking necessary staffing's, following up on items that are required after certain staffing's, contact references for home studies and help resource parents follow up on outstanding items. Support to caseworkers with activities like contacting collaterals, setting up services and appointments, supervising visits, searching for relatives, tracking down records, tracking meetings, scheduling meetings. 75% Documentation in FACTS for Caseworkers. 25% Ideal CandidateExperience as it relates to : Working directly with Children, Youth, and their families; Partner with Community Service providers as it relates to Children, and Youth; Working with Youth in a school setting; Determining eligibility for services that might be needed for the Children, Youth and Families; Conduct and complete assessments of safety, risks, strengths, and family needs; Developing Safety plans for the Children, Youth and Families; Participate in multidisciplinary meetings; Identify, provide, and / or arrange timely assessments / evaluations / interviews; Conduct medical case management activities for eligible clients. Minimum QualificationHigh School diploma or equivalent and two (2) years of experience as a social or community coordinator and / or social worker assistant. Any combination of education from an accredited college or university in a related field and / or direct experience in this occupation totaling two (2) years may substitute for the required experience. Incumbent will be required to furnish a personal vehicle for carrying out assignments which may include transporting clients including children. Must have own mode of transportation.
