With our Agency's mission to support the recovery of people with mental illnesses, we provide a complete array of medical and support services for children, adults, and families throughout South Carolina.This position is with the South Carolina Department of Mental Health, Beckman Center for Mental Health Services, 1547 Parkway Greenwood, SC.The counselor II will serve as a patient are Navigator, developing access resources within the community to assist patients and their family members with self-sufficiency. The incumbent must be able to work with patients from various ethnic and socioeconomic backgrounds with serious mental illness; substance use disorders; and complex medical and social barriers.Responsibilities for the Counselor II include but are not limited to: 1. Assists patients in accessing Primary Health Care, Behavioral Health Care, and Substance Abuse Treatment as needed. Ensures that care is provided in an integrated model, culturally competent and sensitive manner.2. Coordinates e-consults or interprofessional internet consults. This may include but is not limited to coordinating time for e-consults with Primary Care and Behavioral Health; documenting and maintaining information in the registry to include follow-up health measures; routing consult recommendations back to the PCP and facilitating access for PCP to community resources as needed. 3. Serves as member of the Treatment Team. This may include but is not limited to: Coordinating the intake and referral process; collecting preliminary information from patient/family to facilitate intake process; providing referrals to community partners/resources as appropriate and interfacing with families to discuss referrals and recommendations; obtaining validated health measures as requested. 4. Provides direct treatment services and patient education which may include, but is not limited to Psychosocial Rehabilitative Services, Family Support Services, Behavior Modification, Behavioral Health Screenings and other services that the staff member is credentialed to provide. Services will be provided in a culturally sensitive manner and support psychotropic medication management as prescribed by medical providers. Includes development of a relapse prevention plan. 5. Tracks patient progress, program and health outcomes and documents as required and facilitates patient engagement and follow-up care as needed. Ensures outcome measures are completed and given to providers.6. Completes documentation according to QA/SCDMH and SAMHSA standards, complies with Corporate Compliance and Risk Management Plans. 7. Participates in clinical supervision and treatment team meetings and pursues professional development through collegial consultation and continuing education.
