Job Responsibilities Under the supervision of the RPM Coordinator. Determines cadets' requirements by completing intake interviews; review records and application and assure PI information is always secured and properly filed away. Maintain monthly communication with mentors, which may include e-mail, telephone, text or face-to-face contact, to ensure successful mentoring relationships. All contacts must be documented. Maintains cadets' records by reviewing case notes; logging events and progress in the database. All hard copies are to be placed in cadet's file. During the Post-Residential Phase, record placement activities, such as employment, education, military, and miscellaneous (volunteer, caregiver, disabled / hospitalized or incarcerated). Documentation is needed to verify placement activities and placed in the case management file. Document mentoring relationship and cadet placement activities in the data management system no later than the 15th of each month following the reporting period. Manage a case load of multiple cadets. Coordinate services being provided; arranging resources and participate in meetings concerning cadets. Meets with cadets on a rotating schedule to provide personal support, intervene in crisis, and advocate for needed services. Updates job knowledge by participating in educational opportunities; reading professional publications. Enhances department and organization reputation by accepting ownership for accomplishing new and different requests; exploring opportunities to add value to job accomplishments. Other duties as assigned. Minimum and Additional Requirements A bachelor's degree in a related field. A high school diploma and experience related to case management may substitute for the required bachelor's degree. Preferred QualificationsA Bachelor's Degree in Social Work or counseling field and 3 years of relevant experience. Additional Comments Knowledge of agency policies, procedures, regulations and program services. Ability to establish and maintain effective working relationships. Ability to communicate effectively, written and orally. The ability to exercise judgment and discretion. The ability to interpret and apply laws, regulations, policies within the scope of case management. The ability to communicate effectively both written and orally.
