The Washington Corrections Center for Women is actively seeking a Reentry Care Coordinator!! Be a part of our mission to positively improve public safety by positively changing lives through providing service through and with our reentry team.
The Goal of the Reentry Care Coordinator is to provide support with healthcare transition planning as part of the Health Services Reentry Team. This position requires strong interpersonal skills to make connections and build relationships with incarcerated individuals and Department of Corrections (DOC) staff.
This position is performing journey-level work coordinating reentry care and requires strong interpersonal skills to make connections and build relationships with incarcerated individuals and DOC staff. The Care Coordinator works collaboratively with the social worker, nursing staff, the primary care provider, patient service representatives and other team members with the purpose of making the critical linkages to health information/ services and assists with care coordination.
The Reentry Care Coordinator is flexible and willing to work in a dynamic and evolving role according to individual patient and program demands, has strong verbal/written communication skills, and strong organizational skills. To work with the patient and their Health Services Reentry Team to eliminate barriers to continuity of care from prison to community.
**The Washington Corrections Center for Women (WCCW) is a 75-acre facility located approximately two miles from Gig Harbor. The site includes living units, administration buildings, a health clinic, education buildings, a gymnasium, a chapel, industries buildings, food service buildings and support facilities. **

Duties
Develop, Coordinate and implement healthcare reentry planning for patients releasing from prison.

Track releasing patients and their transition of care needs to include managing referrals and triaging health services reentry kites.
- Responsible for organizing multidisciplinary team meetings to include PSW, Reentry Nurse, Provider, Therapist, Classification Counselor, Community Corrections Officer, Reentry Navigator, ICoach Transition Specialist, Family or natural supports and Managed Care Organizations.
- Attend reentry team meetings, wrap around meetings, and other meetings related to transitioning individuals from prison to community.
- Coordinates and facilitates resources to support transition of care to community to include comprehensive and individualized needs as identified by the Health Services Reentry Team.
- Provide Narcan education and other education when needed pertaining to release.
- Partner with Health Services team members to develop processes to ensure individuals have everything they need as they release to include durable medical equipment, release medications, and resource referrals.
- Communicate with Managed Care Organizations when assigned in partnership with HQ Continuity of Care Team and Medicaid Team.
- Provides liaison services between Social Workers/Reentry Nurses and clients. Assists clients in completing forms and applications (to include prison and community programs, benefits and resources).
- Partner closely with clinic team/providers/therapists to ensure Health Services Reentry Team is aware of patient needs during healthcare transition planning.
- Gather documentation needed from health record for Health Services Reentry Team when requested to facilitate access to care, programs, and benefits.
- Update SharePoint sites and evaluate program to include making recommendations to team and supervisor.
- Partner, plan and coordinate with Community Corrections, Prisons, and Reentry Divisions to ensure communication and continuity of care.
- Prepare release packets in coordination with Health Services Reentry Team.

Establishes and maintains community partnerships including internal and external stakeholders to include community providers that take Medicaid to facilitate access to care for patients releasing.
- Eliminate barriers associated with care coordination that might adversely impact patient care.
- Engages physicians, hospital staff, leaders and community partners in analysis, intervention and resolution activities.
- Refer quality, infection control and risk management issues to appropriate individual or department.
- Staffs complex cases with Health Services Reentry team to facilitate timely multidisciplinary case conferences.
- Builds relationships with and understanding of community resources and supports.

Assists patients along their care pathway by serving as an advocate.
- Shares assessments and patient response; remains a support system throughout the patient's course of treatment.
- Provides education and logistical support to improve patient preparedness for treatment and to streamline care transitions to community.
- Provides education and support and ensures that patient understands follow up and referral information.
- Collaborates with the patient, patient representatives and interdisciplinary team to facilitate optimal outcomes as the patient transitions from prison to community.
- Maintains current knowledge of departmental policies/standards/procedures, new clinical procedures/processes, patient safety goals, core measure quality indicators and other issues/requirements impacting matters within assigned scope of responsibility.
- Participate in required trainings and attend department team meetings. Continue education by staying informed of industry trends.
- Develop and maintain processes to improve patient experience and clinical operations.
- Basic knowledge of appropriate boundaries and limits around personal self-disclosure when working as a member of health and support services.
- Strives to create and maintain working and learning environments that are inclusive, equitable, and welcoming.
- Cultural sensitivity and humility demonstrates proper use to communication tools/ materials for effective communication, collaboration and the provision of care, treatment and services. Qualifications
Required Qualifications:
- Two years of experience in a healthcare setting.
- Proficiency using Microsoft Office Software (Word, Excel, Outlook) and SharePoint.
- Excellent written and verbal communication skills.
- Three years of professional experience in adult corrections, social services or closely related field.

Experience with case management for physical and/or behavioral health needs.
Knowledge of community resources.
- Satisfactory completion of two years college or bachelor’s degree in social services related field from an accredited college or university whose accreditation is recognized by the US Department of Education or the Council for Higher Education Accreditation (CHEA).
