DSHS Lakeland Village Habilitation Plan Administrator

Join us at Lakeland Village and help others by providing support and fostering partnerships that empower people who reside here to live a better life. Lakeland Village is seeking a compassionate, experienced, and motivated professional to join our team as a Habilitation Plan Administrator (HPA).

In this role, you will administer the establishment, implementation, and evaluation of client habilitation plans for people with intellectual disabilities. Additionally, you will direct the activities of an interdisciplinary team. An HPA provides habilitative services that support and enhance each client's skills and strengths. Your experience and leadership abilities will make lasting and positive differences.

Note: Opportunities performed on-site at this work location at least three days a week may be eligible for an additional 5% premium pay in addition to the advertised salary.

Some of what you'll do:


  • Develop, write, and consult on training programs for clients.
  • Evaluate and monitor the implementation of habilitation plans for each client assigned.
  • Provide regular analytical review and follow-up of each client's progress in relation to their stated support needs.
  • Provide detailed summaries of health status and/or changes in the lives of clients to families.
  • Recommend appropriate corrective actions when programs are not meeting objectives.
  • Assure compliance of policies, procedures, and practices of the Interdisciplinary Team to Federal and State regulations and laws.
  • Coordinate support needs of people with intellectual disabilities and plan for appropriate services.

Who should apply?
Professionals who meet the qualifications for Qualified Intellectual Disability Professional (QIDP) and one of the following:
  • Bachelor's degree and 1 year of experience in a qualified field*, which has been involved in treating or working with people with intellectual or developmental disabilities.
OR
  • Master's degree
A qualified field may include teaching, social work, working with kids/youth, working with the elderly, or working with people with intellectually/developmentally disabled.

What we're looking for:
  • Experience identifying support needs and planning for appropriate services for persons with intellectual/developmental disabilities.
  • Ability to plan and develop effective teaching, habilitation, and individualized strategies.
  • Ability to prepare clear, concise reports, evaluations, programs, and recommendations.
  • Ability to confront and engage in problem-solving and conflict resolution with clients, team members, and families.
  • Demonstrate good judgment in supporting clients in crisis; ability to recognize and assess the social, emotional, psychological, behavioral, and other factors influencing a client's life.
