CASE MANAGER for Olympic Area Agency on Aging (O3A) based in Aberdeen.Provides case management for in-home care to seniors and adults with disabilities including assessment of care needs, creating service plans, coordinating care providers, and focusing on client independence.Minimum Qualifications : Master’s Degree in behavioral or health sciences & 1 yr. paid social service experience; orBA in similar fields and 2 yrs. paid social service experience; orBA and 4 yrs. paid social service experience.Requires current WDL & insured vehicle.40 hrs. / wk.
