The Developmental Disabilities Administration is looking for a Transitional Case Manager to join our team in Seattle, WA. In this role, you will provide specialized transitional case management and resource assistance for people with developmental disabilities transitioning from one setting to a new one. You will work with families and community members to advocate on behalf of clients.
This position could be based out of the Kent DDA office, as well as Seattle/Cherry Street office. A hybrid telework schedule is available for this role, in our office location in Seattle, WA.

Some of what you will do
- Provide case management, coordination and facilitation of person-centered transition planning activities, which includes transitions from clients residing at the Residential Habilitation Centers, in the hospital and state hospital settings, in their own home in their community, or in a residential setting
- Identify support needs of clients and plan for and authorize payment for appropriate services, i.e., Medicaid benefits, Home and Community Based Services Waiver, Roads to Community Living Program
- Schedule and conduct face to face home visits with clients and families on a regular basis tracking consultation in client database
- Complete and revise client assessments and service plans within established timeline per caseload
- Review and monitor client plans, reports, and client progress of approximately 35 clients
- Refer people to appropriate Developmental Disabilities Administration services, other department services, and community programs/resources
- Collaborate with community organizations and professionals to support clients
- Identify clients or families in crisis and making emergent referrals as needed to appropriate authorities, department/division services, or local community resources
We are looking for professionals who have
- A bachelor's degree in social sciences, social services, human services, behavioral sciences or an allied field, plus 2 years of experience providing social services to people with developmental disabilities
Or
Satisfactory completion of 12 months as a Case Resource Manager Trainee - Ability to identify support needs and plan for appropriate services for persons with developmental disabilities
- Possess a high degree of effective communication skills (write, listen, observe, communicate, interview, understand, confront, engage in problem-solving, and resolve conflict)
- Assess and identify social dynamics between clients and families and/or other relationships
- Ability to identify and notify appropriate entities when clients and/or co-workers may be at risk
