Licensed Master Social Worker (NY HELPS), Mid-Hudson Forensic Psychiatric Center,

Duties Description As a Licensed Master Social Worker, you will be part of a multidisciplinary team that provides culturally competent and evidence-based treatment and support services in a recovery-oriented environment to individual with mental illness.

Duties include :

• Providing crisis intervention, service linkages, and discharge planning
• Developing treatment plans with individuals and treatment team; monitoring and evaluating progress; and modifying treatment plans as needed
• Determining discharge needs and appropriate community resources
• Establishing and maintaining relationships with community providers to ensure continuity of care

NOTE :

If you are appointed and lose the ability to bill through Medicare and Medicaid, you may be terminated from your employment.

Candidates from outside or within State Service can be considered for hire under the Hiring for Emergency Limited Placement - Statewide (NY HELPS) program if they are licensed and currently registered by New York State as a Licensed Master Social Worker (LMSW).
