Duties Description POSITION DESCRIPTION :
Licensed Practical Nurse (LPN) provides nursing services to the Developmental Disabilities population residing in our group homes. The LPNs are responsible to travel within the area of the team as needed.
PRIMARY DUTIES AND RESPONSIBILITIES :
- Monitor the health status of individuals, provide appropriate interventions, and ensure proper documentation.
- Administer medications and treatments.
- Order medications and medical supplies.
- Review and transcribe physician's orders.
- Complete semi-annual nursing summary and attend reviews.
- File medical reports, ensure follow-up and tracking of medical appointments, and maintain medical record.
- Supervise and provide direct care services, including ADLs.
- Provide and coordinate in service training for Direct Support Assistant Trainees.
- Personal care of consumers residing in the home will be required.
- Monitor the conditions of the Individuals in their assigned worksite in order to detect indications of changes in condition or responses to medication or therapy.
- Identify symptoms of illnesses and changes in conditions manifested by individuals and report such symptoms or changes to a physician or registered nurse in order to provide information useful in diagnosis and treatment planning.
- Take temperatures, pulse rates, blood pressures and respiration rates using thermometers, manometers, stethoscopes.
- Administer all injectable, j-tube and g-tube medications to assigned individuals.
- Implement and monitor the g-tube and j-tube feedings, including changing g-tubes as necessary.
- implement catheterization orders, trach care, deep oral and pharyngeal suctioning.
- Assist individuals in collecting urine, stool, and other specimens in order to obtain such samples for analysis.
- Prepare and maintain charts and other records and treatments administered in order to document such conditions and activities.
- Perform inventory of current medical supplies and medications to determine the need for ordering new supplies or medications.

Minimum Qualifications MINIMUM QUALIFICATIONS :
Either
A : A license and current registration to practice as a practical nurse in New York State,
OR B : A limited permit to practice as a practical nurse in New York State,
OR C : An application on file for a limited permit to practice as a practical nurse in New York State. (You must be approved immediately following graduation from an approved school of practical nursing in New York State to qualify under this option.) Note : If you are appointed with either qualification "B" or "C," you must obtain your license within one year of the date your limited permit was issued or steps will be taken to remove you from the position. This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title. At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.
