Duties Description Under the supervision of the Quality Improvement Department MHPES 4, the Rehabilitation Counselor 2 will function as the Incident Review Coordinator. The incumbent will be responsible for ensuring that incidents are reported Uploaded into Incident Report Management Application (IRMA) and completed with in accordance with the 624 Regulations and Jonathan's Law requirements.
Responsibilities include :
1. Interface with LIDDSOO staff, Incident Management Unit (IMU), Office of Internal Investigations and Affairs (OIIA) and the Justice Center.
2. Uploading all necessary incident documents into IRMA.
3. Maintain spreadsheets for tracking incidents, completed Corrective Action Plans, Immediate protections, Incident Recommendations, Completed Agency level investigations.
4. Chair the Incident Review Committee (IRC), and approve, submit, and upload IRC Minutes.
5. Chair the IRC Sub Committee
6. Provide support to the Administrative Assistant and Social Worker Assistant 1
7. Review Trend reports for the Agency.
8. Assist with questions regarding incidents.
9. Provide training to the units on incident reporting.
10. Review and approve redacted incidents.
11. Close out incidents
12. Sending out Notifications
13. Request documents from the units
14. Assigning Incidents to investigators
15. Special assignments such as litigation requests
16. Other duties as assigned by supervisor.

Minimum Qualifications PROMOTION :
One year of permanent service as a Rehabilitation Counselor 1 AND a qualifying bachelor's or higher degree

OPEN COMPETITIVE QUALIFICATIONS :
A bachelor's degree in a human service field and three years of professional experience providing vocational, psychosocial, or rehabilitative / habilitative services in an agency primarily serving individuals diagnosed with developmental disabilities, mental illness, or addictions.
Substitution :
A master's degree in a related field can substitute for one year of specialized experience.
Additional Comments This title is part of the New York Hiring for Emergency Limited Placement Statewide Program (NY HELPS). HELPS Program titles may be filled via a non-competitive appointment. This means that you do not need to take an exam to qualify, but you do need to meet the minimum qualifications of the title.
At a future date (within one year of permanent appointment), employees hired under NY HELPS are expected to have their permanent non-competitive employment status converted to permanent competitive status. You will not have to take an exam to gain permanent competitive status.
