Staten Island Performing Provider System (SI PPS) is a non-profit organization participating in the New York State Department of Health 1115 Medicaid waiver amendment, New York Health Equity Reform (NYHER). Under the waiver, SI PPS is designated as a lead entity to support the Richmond County Social Care Network (SCN). The SCN lead entity is responsible for coordinating a network of community-based organizations and providers to better enable health-related social needs (HRSN) screening and the delivery of social care services. The overall goal of the program is to improve health outcomes, provide equitable access to care, and reduce health disparities.
Job Description: Reporting to the CBO Network Manager, the Outreach & Engagement Coordinator is responsible for engaging with, training, troubleshooting, and monitoring the performance of Social Care Navigators/CHWs working at community-based organizations and physician practice sites who will be providing health-related social needs (HRSN) screening, navigation and the delivery of HRSN services. The Outreach & Engagement Coordinator will be responsible for understanding the Social Care Network technology platform and assist the training lead with technical assistance for Social Care Navigator/CHW users. The Outreach & Engagement Coordinator will work closely with the Field Operations Manager, the CBO Network Manager and the Community Engagement Coordinator to use the platform to closely monitor screening and referral metrics ensuring data quality and completeness of screenings and documentation and ensuring the HRSN service directory is updated. The role involves approximately 80% field work to community-based organizations and approximately 20% office time for follow up and team meetings.
Responsibilities:
- Build and foster relationships with Social Care Navigators/CHWs, community-based organizations and physician practices that will be contracted to deliver social care services to Medicaid members in the Social Care Network
- Assist with training community-based organization users on the Social Care Network technology platform, track training deliverables and goals and monitor performance of new users
- Troubleshoot technical issues and support Social Care Navigators/CHWs in the field; escalate clinical, navigation or performance concerns to CBO Network Manager
- Provide quality assurance and monitoring of metrics extracts and reports from the Social Care Network platform
- Collaborate and communicate with team members, partner-based navigators/CHWs, and community-based organizations to monitor and improve screening rates and completion of demographic and insurance information, fully completed screenings and closed-loop referrals
- Visit partner sites and other community-based screening locations to check-in on performance, goals, and services
- Strong understanding of Social Care Network program requirements.
- Communicate critical updates and provide reports to leadership regarding successes, challenges and outcomes
- Assists with collection of information regarding the delivery and outcomes of services.
Qualifications:
Education/Experience:
- Bachelor’s Degree in related health, social services field highly preferred. Significant and documented relevant work experience may be accepted in lieu of education.
- 3 years of experience engaging and organizing community-based organizations or building community-based service networks required; program management experience preferred
- Extensive knowledge and understanding of social determinants of health, community-based organizations (CBOs), health equity, health care systems, and value-based purchasing (VBP) a strong plus
- Demonstrated experience preparing and presenting proposals, reports, updates, data, summaries, etc.
- Experience with City and State public benefit programs and use of designated portals and application systems
- Experience using health information systems or other databases preferred
