CAREGIVER
Reports to: Wellness Director / Wellness Coordinator
The Caregiver provides to each Resident within The Manor Village at Desert Ridge their selected and personalized wellness care needs and services. The Caregiver assists each Resident in adequately performing activities of daily living (ADL), including specific interventions as outlined in the individual wellness plans. The Caregiver works closely with the Inter-disciplinary Wellness Team in maintaining a positive physical, nutritional and social environment for each Resident, while promoting safety and enhancing the quality of life for each Resident. The Caregiver is expected to carry out all roles and responsibilities consistent with the philosophy and Core Values of The Manor Village Life Centers and, must be ready and willing to respond to the emergency needs of all Residents in a timely and professional manner.
Requirements and Accountabilities
Resident Lifestyle Services
- Ensure aids to daily living have been satisfactorily completed
- Document each Resident assignment appropriately and charge for all pay-for-fee services provided
- Assist each Resident with their medication (if qualified) and document per policy/guideline
- Measure and accurately record vital signs (temperatures), pulse, respiration, blood pressure, weight and height) as per The Manor Village at Desert Ridge guidelines and wellness plan
- Check on each Resident daily for effective self-care (cleanliness, appropriate clothing, continence issues) and assists accordingly
- Verify that charting is complete as scheduled and check for other noted events (i.e. admissions)
- Communicate and document each Resident material needs (i.e. briefs, clothing, toothpaste, incontinent products)
- Assist with meal times and or serving of meals as directed
- Ensure that the personal belongings of each Resident are handled carefully
- Inform the Team Lead LPN and/or Wellness Director of changes in the condition and wellness needs of Residents
Teamwork
- Relieve colleagues who are on breaks
- Maintain duties as per shift roster/daily work schedules

- Ensure that co-workers are offered assistance as needed in the course of each shift

- Participate in shift exchange with colleagues
Analysis/Reporting
- Receive and give report of on-going shifts by oral and/or written log
- Report shift updates in communication book
- Observe and report any significant changes in Resident behavior, health status or needs
- Report all Resident and family concerns and issues
- Document each Resident assignment / core task appropriately
- Report ALL Resident incidents, altercations or unusual behavior and advise the Team Lead Caregiver and/or Wellness Director of any concerns as soon as possible
Compliance and Safety
- Respond to all emergencies as directed
- Ensure soiled clothing is placed in yellow bag and labeled before transport to common area laundry (if applicable)
- Check on Resident rooms daily for general cleanliness and neatness; ensure high risk rooms are double checked
Activities
- Participate in activities and assist staff and Residents in daily recreational programs and activities as directed
Facilities Maintenance
- Assist in general cleaning of spills/soilage
- Report any malfunctioning equipment appliances, fixtures, etc. to maintenance and/or Team Lead Caregiver and/or Wellness Director
- Help to keep Common Areas presentable and in order
- Do not leave any in-use equipment unattended
- Check laundry rooms, Common Areas, including bathrooms, for cleanliness
Qualifications
Qualifications & Skills
- Must have successfully completed a caregiver training program approved by the Department or the Board of Examiners for Nursing Care Institution Administrators and Assisted Living Facility Managers (Arizona)
- Demonstrate knowledge OSHA guidelines (e.g., chemicals, personal care products, and items locked)
- Must possess initiative and judgment and ability to maintain confidentiality in keeping with the Health Insurance Portability and Accountability Act (HIPAA)
- Demonstrate working knowledge of standards and regulations as they pertain to the job
- Must have complete skill set to accomplish all outlined duties
Physical and Sensory Requirements
(with or without the aid of mechanical devices)
- Must be able to walk and stand for multiple hours in duration
- Ability to stand, walk, stoop, kneel, or crouch for prolonged periods
- The ability to turn, stoop, bend, stretch, and lift to assist residents in meeting their wellness needs and other related duties.
- Use of hands to finger, handle or feel objects, tools or controls, with each hand/arm
- Medium Physical Demand Level: ability to regularly lift and/or move up to 9 kg / 20 pounds, and occasionally lift up to 23 kg/50 pounds
- Good visual acuity
- Good hearing acuity
- Mental acumen
- Must have minimum three (3) months experience
Working Conditions
- Must possess a high degree of flexibility and tolerance to adjust to changes in workloads and days (i.e. weekends, evenings etc.)
- Prolonged standing and walking
- Lifting and assisting in transferring of residents,
- Lifting equipment and supplies
- Repeated bending and stooping
- Must be able to prioritize tasks and following procedures to meet the wellness needs of each Resident

Salary : $19 - $21
