What are the responsibilities and job description for the Intake Coordinator position at StayWell Senior Care Center?
The Intake Coordinator is responsible for facilitating the admission process for all potential participants. Act as the liaison between the participant, their family members and the StayWell team up to the date of admission. Coordinate the assessment schedule for the Interdisciplinary team. Bachelor’s degree in nursing, social work or other related field required. Must possess knowledge of the principles of effective public relations and communications; mass media, publication production; community relations; and promotions. Must possess the ability to write with clarity, accuracy and speed. Must have effective public speaking skills. Must possess the ability to assimilate information from a variety of sources and analyze information correctly.
One year experience in senior service, health care and/or assessment required.
