Established in 2018, Steadfast serves 17 people in shared living settings across the St Louis metro area. Position Summary
The Team Leader is responsible for coordinating comprehensive services for individuals with Intellectual and Developmental Disabilities in a community living environment. The Team Leader works closely with companions, direct care staff as well as host home providers to ensure that each individual’s person-centered plan in being implemented. The Team Leader is responsible for developing and implementing all facets of home life as close to a normal family unit as possible. The Team Leader works closely with the Residential Director to ensure companions, direct support professionals and host home providers receive training and build the necessary skills to successfully support the individuals the agency works for.
Continued employment requires successful completion of all training as required by the Missouri Department of Mental Health, Developmental Disabilities, and Steadfast.

Work Hours
The Team Leader’s hours of work will vary depending on the needs of the people supported. The Team Leader must be flexible enough to accommodate participant, companions, host home providers, and/or staff’s schedules, while also allowing for time in the office or work environment to meet paperwork expectations. The Team Leader must be able to work weekend and evenings, as it is necessary, and being responsive to crisis and emergency situations is an integral part of the position. The Team Leader is allowed flexibility in their work schedule, but it requires prior coordination and approval. As a general rule, the Team Leader should spend 40% of their time in the office environment while the other 60% of their time with participants, companions, staff and/or host home providers in their homes.
On Call Procedures
The Team Leader is expected to fulfill on-call duties as assigned. Typically, the Team Leader must remain available to individuals, companions, host home providers and/or staff during all hours from Sunday at 3pm until Friday at 3pm. When unable to fulfill this requirement, the Team Leader must take proactive measures with their team and the Residential Director to ensure coverage is maintained. Weekend on-call duties (Friday at 3pm to Sunday at 3pm) are rotated among a team of other members of leadership and/or the Residential Director. Answering work related calls during times when not assigned on-call duties is highly discouraged. The Team Leader should work closely together with his/her team to ensure coverage is maintained.
Essential Duties and Functions
The following is a list of job duties that are viewed as “Core Responsibilities”. “Core Responsibilities” are essential job functions of a Team Leader, must occur and are considered non-negotiable. The Team Leader that fulfills these job functions are meeting minimum standards related to job expectations.
· Maintain ongoing and positive communication with participants, companions, staff, guardians, parents, host home providers, and Service Coordinators about policies, issues and/or concerns.
· Actively participate in the person-centered planning process (e.g. attend meetings, providing input, coordinating the development of plans, completing assessments, and providing feedback).
· Attend transition meetings in partnership with the Service Coordinator when individual is transitioning to community setting.
· Complete, train and implement the Positive Behavior Support Plans and Rights Restrictions (within the first 30 days of initiation of services and annually or as warranted thereafter) when applicable.
· Attend Human Rights Committee and Behavior Intervention Committee meetings regularly and provide input and prepare plans to be reviewed as necessary.
· Complete Emergency Rights Restrictions document and notify appropriate committee members, guardians/parents, and Division of Developmental Disabilities and Residential Director.
· Assist in accessing monitoring documentation for Service Coordinators and other persons as applicable.
· Ensure that the person-centered plan is being implemented as written and is communicate with Service Coordinator when the plan needs to be changed or updated.
· Review of companion and host home documentation to ensure completion and accuracy (e.g. progress notes, training plans, daily notes, emergency drills, MARs, etc.).
· Coordination and oversight of medical appointments, including attending important or necessary appointments as needed.
· Coordination and oversight of participants to be healthy and safe in their pursuit of a meaningful desired lifestyle. Promote dignity of risk for participants by supporting participants to have an appropriate balance between what is important “to” them and what is important “for” them.
· Conduct face to face visits with individuals living in ISL, companion and host homes, complete required site documentation for each visit.
· Ensure that each individual’s rights are respected and protected. Advocate on behalf of the individuals served.
· Oversight and maintenance of participant records to ensure that data is current and comprehensive. Reinforce expectations with companions, host home providers and DSPs.
· Monitor individuals for safety, including ensuring that each home environment is safe, clean and in good repair.
· Provide appropriate ongoing feedback, both formal and informal to companions, host home providers and DSPs.
· Participate in evaluation process of vetting all companions, host home providers and DSPs.
· Involve participants to the greatest extent possible in directing their services and supports.
· Assist companions and host home providers in finding respite/relief when needed.
· Participate in incident reporting management including review of all incident reports, making appropriate and required notifications timely to Residential Director, Division of Developmental Disabilities, Adult Services and Aging and/or Local Law Enforcement. Provide oversight to reports that would require further follow up actions to appropriate parties. Attend Incident Management meetings as needed.
· Reinforce and ensure companions, host home providers and DSPs comply with Health and Insurance Portability Act of 1996 (HIPAA).
· Fulfill on-call duties as assigned. When unable to fulfill on-call responsibilities, take proactive measures to ensure on-call functions and coverage is maintained.
· Be available and responsive to companions, host home providers, and DSPs in emergency and crisis situations.
· Communicate and collaborate with the Residential Director to ensure continuity of supports and services. This includes ensuring that other members of leadership are adequately prepared to assume on-call responsibilities for your workload when you are unable to fulfill these expectations.
· Perform other work-related work or duties as assigned by Residential Director.

While not necessary a “Core Responsibility”, the Team Leader that fulfills these job functions exceed minimum standards.

- Look for opportunities to build teamwork and collaboration between companions, host home providers, DSPs and individuals served.
- Discover ways to increase and improve communication with participants, families, Service Coordinators, guardians, companions, host home providers and DSPs.
- Promote healthy living with individuals, companions, host home providers and DSPs by providing resources and information about healthy options and alternatives.
- Assist trainer in notifying staff of training needs and expectations.
- Connect individuals with opportunities to further their knowledge of self-advocacy and actively directing their services and supports.
- Assess the need to intervene and provide assistance in resolving conflict between companions, host home providers, DSPs, parents/guardians and the individuals.
- Discover opportunities for individuals served to take active and valued roles within their communities.
- Provide emotional, caring, and reassuring support to companions, host home providers, and DSPs.
Qualifications
Bachelor’s degree in Human Service field, including but not limited to sociology, special education, rehabilitation, nursing, counseling, and psychology; is preferred. Candidates with Master’s degrees will be highly considered. At least five years’ experience providing support to individuals with intellectual/developmental disabilities, mental health and/or in human service organization; and at least two years with supervisory experience. Experience with Person Centered Practices is highly preferred. Must also be willing to work flexible hours, including days, evenings, nights and weekends.
