What you'll do
You’ll shape this role for countless future Hera Heroes to follow in your footsteps.
- serve older adults with dementia as a dedicated care navigator by collaborating with their PCPs to address health-related social needs
- build an internal knowledge base of community resources and relationships with local community organizations
- help set up Medicare documentation processes with our consultants
- establish training for future Hera Heroes and potentially become a supervisor for the network
- provide input to building a software platform we’ll be building to supercharge Hera Heroes
