What are the responsibilities and job description for the Care Coordinator/Case Manager or RN or PHN position at STEELE COUNTY?
The Care Coordinator/Case Manager on the Senior Team serve as both case manager and care coordinator for persons age 65 and above who reside in Steele County. Persons served are eligible for Essential Community Supports, Alternative Care or Elderly Waiver. The focus is the health and safety of persons served. Staff work to assess needs, develop plans that reflect the individual’s goals and assist in the acquisition of services and supplies needed to meet individual goals. The case manager will be responsible for conducting assessments, creating support plans, as well as collaborating with other members of the care team.
Working in the Public Health Department, other job responsibilities will include a broad classification of professional public health positions which are responsible for improving the health of individuals, families, groups and the community by providing:
- Providing health care treatments and interventions in accordance with care plans
- Assessing and evaluating the home environment to identify care/health issues and revise care plans
- Providing instruction and education to clients regarding how to maintain the least restrictive environment and how to adjust to various physical, mental or social limitations
- Participating in various immunization/vaccination clinics
- Participating in any emergency preparedness events, as needed
- Other duties as assigned
