What are the responsibilities and job description for the RN or PHN position at STEELE COUNTY?
The RN or Public Health Nurse will be a part of the Long Team Care team and serve as a care coordinator for persons age 65 and above who reside in Steele County. Persons served are eligible for Medical Assistance through one of the health plans in Steele County. The focus with the position is the health and safety of persons served. Staff will work with clients to assess needs, develop plans that reflect the individual’s goals and assist in the acquisition of services and supplies needed to meet individual goals. The person who fills this role will be responsible for conducting assessments, creating support plans, as well as collaborating with other members of the long-term care team.
Working in the Public Health Department, other job responsibilities will include a broad classification of professional public health positions which are responsible for improving the health of individuals, families, groups and the community by providing:
- Health care treatments and interventions in accordance with care plans
- Assessing and evaluating the home environment to identify care/health issues and revise care plans
- Instruction and education to clients regarding how to maintain the least restrictive environment and how to adjust to various physical, mental or social limitations
- Participating in various immunization/vaccination clinics
- Participating in any emergency preparedness events, as needed
