DEPARTMENT: Supported Housing Counselor – Jamestown, NY - Chautauqua County
SCHEDULE: Monday through Friday, – 9:00 am to 5:00 pm
SALARY: Minimum $19.57/ hr.
AVAILABLE: ASAP
JOB FUNCTION:
Provides case management, and support services to consumers. To teach, counsel, and monitor consumers’ activities of daily living in a supported housing program. To establish all necessary linkages to any required services that help consumers gain self-sufficiency, become integrated into the community, and prevent a consumer from becoming homeless.

MINIMUM QUALIFICATIONS:
Education, Experience, and Knowledge
Bachelor’s degree in human services-related field; OR associate degree in human service-related field and three years’ experience in human services; OR a high school diploma and five years’ experience in human services.
Must possess a working knowledge of current issues and concerns of people with mental illness, rehabilitative training methods, and community resources.
OTHER QUALIFICATIONS:
- Have a home telephone.
- Be able to travel to and among various facilities that are multileveled with stairs only.
- Have a valid driver’s license and a safe driving record.
- May be required to use personal vehicle for programmatic purposes.
- Criminal Background check approved by NYS Office of Mental Health.
