JOB DESCRIPTION: Ambulatory Withdrawal RN

IMMEDIATE SUPERVISOR: Outpatient Nursing Manager

POSITION SUPERVISED: None

QUALIFICATIONS/CREDENTIALS:

1) Current Ohio Licensure as a Registered Nurse

2) Current CPR certification

3) One year experience in the Behavioral Health care field

BASIC FUNCTION:

Under direction of an APRN or PA, provides direct patient care to patients who are detoxing from Alcohol and Drugs, following established standards of practice, department guidelines and existing policies and procedures.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:

  • Performs accurate and complete admission assessments prior to client starting programing. Assesses patient for sign and symptoms of post- acute withdrawal, safety, abuse and neglect for all clients transitioning into PHP and Ambulatory withdrawal.
  • Assess documents and manages patient’s pain using appropriate pain scale.
  • Prioritizes and documents patient problems based on assessed needs/problems in collaboration with patient, family and healthcare team.
  • Assess all clients in the ambulatory detox program every morning for signs and symptoms of Post-Acute Withdrawal. Complete Paws assessment and report to morning huddle.
  • Discharges and admits clients to the correct program based on their PAWS scores and medications.
  • In collaboration with Nursing Supervisor and Nurse Practitioner manage and discontinue all medications
  • Medicate all clients in the ambulatory detox program for the daily dosing schedule.
  • Work with Nursing Manager and Nurse Practitioner to manage medications and refills for all clients attending PHP and in Ambulatory Detox program.
  • Education all clients on ambulatory detox medications and interactions.
  • In collaboration with the interdisciplinary team, assist in the discharge and transfer of all client’s medications and MARs to the next level of care.
  • Under the direction of the Nursing Manager, helps to plan the delivery of patient care in collaboration with other disciplines to promote wellness and facilitate the discharge planning process.
  • Documents changes in patient condition and effectiveness and timeliness of care/interventions.
  • Sets up appointments with the Outpatient Nursing Manager or NP as needed to manage clients medical and mental health needs.
  • Assesses educational needs of patients and significant others considering cultural issues and other special challenges.
  • Provides teaching and educational material to the patient and/or significant other concerning the following topics: diagnosis, reason for and safe use of medication, food/drug interactions, injury prevention, nutritional needs, HIV, TB, Hepatitis and STD.
  • In collaboration with the Nurse Manager and Nurse Practitioner provide quarterly education to the Resident Assistants in supportive housing surrounding all aspects of medication pass and documentation.
  • Works with the Nurse manager to ensure that all medication carts in PHP and supportive housing are in proper order and meet joint commission standards.
  • Validates and documents patient and family understanding of teaching.
  • Utilizes the 5 rights of medication administration.
  • Knows action, usual dosage, indications and side effects of medication administered.
  • Under the direction of the Nursing Manager, utilizes the correct reporting mechanism for any medication errors, near misses, etc.
  • Verbalizes understanding of 12-step model of addiction recovery.
  • Demonstrates ability to effectively de-escalate patient crisis situations.
  • Demonstrates efficiency in use of automated medical record.
  • Completion of requirements for annual agency training compliance.
  • Ability to uphold patient rights and confidentiality regulations.
