Position Description

JOB DESCRIPTION: LPN or RN

IMMEDIATE SUPERVISOR: Director of Nursing

POSITION SUPERVISED: None

QUALIFICATIONS/CREDENTIALS:
1) Current Ohio Licensure as a Licensed Practical Nurse or Registered Nurse
2) Current CPR certification
3) One year experience in the Behavioral Health care field

BASIC FUNCTION:
Under direction of an RN, provides direct patient care to patients who are chemically impaired, following established standards of practice, department guidelines and existing policies and procedures.

ESSENTIAL JOB DUTIES AND RESPONSIBILITIES:

1. Under the direction of the RN, performs accurate and complete admission assessments/reassessments within specified time frame. Assesses patient for safety risks, abuse and neglect.

2. Assesses, documents and manages patient’s pain using appropriate pain scale.
3. Prioritizes and documents patient problems based on assessed needs/problems in collaboration with patient, family and healthcare team.
4. Under the direction of the RN, plans delivery of patient care in collaboration with other disciplines to promote wellness and facilitate the discharge planning process.

5. Documents changes in patient condition and effectiveness and timeliness of care/interventions.

6. Assesses educational needs of patients and significant others considering cultural issues and other special challenges.

7. Provides teaching and educational material to the patient and/or significant other concerning the following topics: diagnosis, reason for and safe use of medication, food/drug interactions, injury prevention, nutritional needs, HIV, TB, Hepatitis and
STD.

8. Validates and documents patient and family understanding of teaching.
9. Utilizes the 5 rights of medication administration.
10. Knows action, usual dosage, indications and side effects of medication administered.

11. Under the direction of the RN, utilizes the ADR reporting mechanism for any medication errors, near misses, etc.

12. Verbalizes understanding of 12-step model of addiction recovery.
13. Demonstrates ability to effectively de-escalate patient crisis situations.
14. Demonstrates efficiency in use of automated medical record.
15. Completion of requirements for annual agency training compliance.
16. Ability to uphold patient rights and confidentiality regulations.

NON-ESSENTIAL DUTIES:

1. Performs other duties of the position and within the individual’s capabilities as assigned by his/her supervisor.
