Responsibilities
The essential function of this position is to provide hospice nursing and palliative care to terminally ill patients. The hospice nurse will ensure the highest level of care is provided to our patients as they transition to end-of- life care. Professionalism and compassion are essential to success in this role.
Performs daily assessments of patients’ physical and emotional needs with appropriate intervention and management of symptoms and coordination of other disciplines.
Enters all required documentation into EMR to include daily clinical notes, effectiveness of interventions/treatment plan, updating care plan, updating medications when necessary, and assessment if indicated
- Administers medications in accordance with policies and procedures and in compliance with federal and State regulations. Follows the 7 Rights of Medication administration.
Consistently demonstrates ability, accuracy and knowledge for transcribing physician’s orders to MAR. Completes monthly turnover of MAR's.
Completes accurate narcotic counts at shift change and properly disposes of controlled medications upon patient’s death or discharge following DEA regulations
Completes 24 hour chart check for accuracy of orders and transciption.
Must pass an annual medication test.
Completes an accurate and thorough shift report.
Notifies MD/CRNP of change in condition of patients.
Completes comprehensive admission assesments on all new admissions.
Completes documentation for weekly Team Meetings. Participates in weekly Team meetings provideing accurate and current information on the patient.
Demonstrates compentency in the use of IV -Central & Peripheral lines, sub - cutaneous therapies, IM injections, PICCs, PORTS, basic TPN/PPN, and PCA pumps.
Demonstrates competency in ostomy, drains, foley catheters, macy catheters, and wound care.
Demonstrates competency in trach care, CPAP, BiPAP, and high-flow oxygen.
Competent in the ability to assess, implement and evaluate respiratory issues and treatments.
Demonstrates competency with G-Tubes, NG Tubes, and tube feedings.
Follows infection control policy and procedure.
Demonstrates good body mechanics.
Delegates patient care tasks appropriately.
Supervises and provides direction to the aides on the care delivered. Provides feedback for the aides' annual performance appraisals .

Requirements
• RN licensure in the State of Maryland
• Current Cardio Pulmonary Resuscitation (CPR) certification
• Prior experience in hospice required
• Certified Hospice and Palliative Nurse (CHPN) a plus
