Summary The Referral coordinator is an integral part of the care coordination team and is responsible for collaborating with patients, families, caregivers, physicians and other members of the multidisciplinary team to coordinate and help manage patient needs. The intent is to ensure that patients are able to access all appropriate services in a timely and cost effective manner. The Referral Coordinator will work to overcome obstacles and guide members through the multidisciplinary systems.
- This role requires someone who has empathy, compassion and an understanding of the cultural and socioeconomic backgrounds in the communities and is knowledgeable about community resources, including financial, educational, social, and emotional support services available to patients.
- The Referral Coordinator will work closely with providers from both Pediatrics and Internal Medicine. This position will, in some cases, act as the liaison / patient advocate from the practice into the community based on the need. Requirements EDUCATION AND WORK EXPERIENCE Education and Work Experience : High school diploma or GED Bachelor Degree preferred or 5 years of experience in related field Communication Skills (written, verbal, presentation and patient engagement) Interpersonal and Relationship Building Skills (with patients and medical staff) Service Coordination and Navigation Skills Health Care Knowledge Base 1 year experience in health care / case management Excellent records tracking skills SPECIFIC REQUIREMENTS Knowledge, Skills and Abilities : Microsoft Word, Power Point, Excel Microsoft Outlook Meditech (if applicable to position) Epic (if applicable to position) Performance Manager / NetLearning Computer proficiency and technical aptitude with the ability to utilize one or more of the following : Must be able to read, write, and speak the English language in an understandable manner. Must be able to work independently and possess the ability to make independent decisions when circumstances warrant such action. Must possess the ability to effectively communicate and deal tactfully with other personnel, visitors, patients, physicians, customers, and the general public. Must be flexible, have personal integrity, and the ability to function effectively as a team with other employees. Must be able to follow written and oral instructions. Other Requirements : Must be able to accurately type 40 words per minute. Must have clinical skills.
