Stellar Care and Services is hiring for a Coordinator in our Supported Employment program in Colorado Springs. We provide employment support services to those who qualify through the Division of Vocational Rehabilitation and those who are eligible through HCBS-SLS and HCBS-DD waiver services.

Main Job Tasks and Responsibilities
- Consistently exercises individual judgment, makes decisions and solves problems.
- Works with Program Director to review individual referrals, meet with family/guardian to sign up the individual to Stellar Care services and conduct orientation with individual and families/guardians as needed.
- Create monthly progress reports to submit to DVR that include services provided and challenges the individual may face.
- Completes daily notes of individual’s progress and challenges, timesheet and goal tracking.
- Develops schedules for job coaches whom this position directly supervises.
- Works with Program Director to update and maintain program participant records.
- Investigates complaints or performance concerns; manages conflicts and disagreements in a constructive manner; responds to grievances; identifies and solves individual’s problems.
- Represent Program Director, Director or designee at internal and external meetings.
- Builds resumes, schedules interviews with potential employers and conducts mock interviews
- Coaches real-life job skills to individuals with disabilities at their workplace while providing the needed supports to help them obtain and maintain meaningful employment in the community.
- Provides on-site training, instruction, supervision, and advocacy for individuals at community job sites
- Maintains and distributes employment information such as job openings to Program Director and individuals.
- Support individuals in increasing independence in their workplace and community
- Assist individuals in understanding the functions and additional tasks of their job.
- Assist individuals in communicating and cultivating meaningful relationships with coworkers and management within their job and professional community.
- Assist individuals in advocating for their needs while at work, including potential formal accommodations needed.
- Maintain proper community supervision requirements.
- Transportation to and from work/interviews if required.
- Provides medication administration to individuals served when needed under QMAP certification.
- Proactively performs outreach to cultivate relationships with perspective employers.
- Maintains a healthy relationship with individuals serving as their single point of contact as needed; checks in regularly with the individual throughout their employment and connects them any supports or resources needed through job placement.
Qualifications:
- Must have reliable transportation with a vehicle that can pass a multi-point safety inspection through a certified mechanic (Stellar Care covers the cost of the initial inspection).
- Must have a valid Colorado driver's license, and a willingness to transport clients in your vehicle.
- Must be at least 18 years of age.
- Must be able to work evenings and weekends as needed, though office hours are 8a-5p Monday through Friday.
- Able to perform the physical and mental demands of the position as the environment will shift between in office demands and on the job supports for people served.

Schedule: Schedule will vary depending on job schedules of individuals. Evenings and weekends may be required. On call rotation one weekend every six weeks is also required.
