Scope

The range of Nurse Health Case Management and Delegation Services include, but shall not be limited to:

  • Performing a comprehensive nursing assessment of the individual identifying his or her health, medical appointment and nursing needs;
  • Review of the Health Risk Screening Tool (HRST) at Level 3 or above, both on an annual basis and when any significant changes in the health of the individual occurs, to assist the individual and the team to understand his or her health needs and to make recommendations to the individual and the team for obtaining services in the community;
  • Completion of the DDA Medication Administration Screening Tool, minimally annually and when any significant changes in the cognitive status of the individual occurs, to determine or verify the level of support needed for medication administration;
  • Review of the individual’s health services and supports delivered by the DDA provider agency direct support staff for safe, appropriate and cost-effective health case as per Maryland Board of Nursing (MBON) definition of case management;
  • Providing recommendations to the individual, caregivers under delegation of the RN, and the team for the health care services that are available in the community.

“Nursing Services provides participants a licensed Registered Nurse (the “RN Case Manager & Delegating Nurse” or “RN CM/DN”) who: (1) provides health case management services (as defined below);

and (2) delegates nursing tasks for an unlicensed individual to perform acts that may otherwise be performed only by a RN or Licensed Practical Nurse (LPN), as

appropriate and in accordance with applicable regulations”. Services is primarily provided in a residential setting and included in rate for 45-day reviews.

  • Communicating with the individual and his or her person-centered planning team members in order that the team can coordinate services and supports to meet the individual health needs;
  • Develop health care plans and protocols ,as needed, that direct the paid direct support staff in the provision of health services to be performed that include (a) administration of medications, (b) performance of medical and nursing treatments, (c) activities of daily living (ADL) performance, and (d) identifying and intervening in an emergency;
  • Completionoftraining,supervision,evaluationandremediationonallhealth services provided under the delegation of the RN by the paid staff as identified in the Nursing Care Plan.
  • MonitoringservicesdeliveredunderdelegationoftheRNbydirectsupportstaff for compliance with the Nursing Care Plan; and,

10.Monitoring health date obtained by direct support staff under the delegation of the RN and as directed in the Nursing care plan.

11.Assessment of (a) the needs and abilities of the individual; (b) direct care staff performance of delegated nursing task; and (c) the environment of service or care delivery;

12.Delegation of the performance of nursing tasks (i.e., acts of a licensed nurse that include medication administration and treatment administration) to unlicensed direct care staff that may be Certified Medication Technicians (CMT), Certified NursingAssistants (CNA) , or other unlicensed assistive personnel (UAP) in accordance with applicable Maryland Board of Nursing regulations;

13.Training, supervision, and remediation of unlicensed direct care staff who provide health services under the delegation of the RN. (e.g., administration of medication, treatment and ADL care, health monitoring) as required by applicable Maryland Board of Nursing regulations; and, Provision of on-call service, to paid direct support staff that are performing delegated nursing tasks, while delegation is occurring, for up to 24 hours per day, 365 days per year as required by applicable Maryland Board of Nursingregulations.

Staffing and Training

Role 1: Nursing Services

All staff and care providers receive adequate training to perform their assigned duties; All staff and care providers successfully complete the administration's approved training to meet the specific needs of the individuals they serve and to carry out their assigned duties, such as training in: seizure disorders, principles of behavior change, management of disruptive behaviors, and the aging process and the special needs of the elderly.

All staff and care providers receive training in blood-borne pathogens in accordance with OSHA guidelines found in 29 CFR §1910.1030, which is incorporated by reference.

All staff and care providers shall receive administration-approved training within 3 months of hire in the following: Community integration and inclusion, Individual-directed, outcome-oriented planning for individuals, General characteristics and needs of individuals served, first aid and cardiopulmonary resuscitation (CPR), based on the guidelines of the American Red Cross or another nationally recognized organization, fundamental rights of individuals with developmental disabilities, communicable diseases, supporting individuals and families in making choices, and communication skills.

A person who has a criminal history, which would indicate behavior potentially harmful to individuals, documented through either a criminal history records check or a criminal background check, pursuant to Health-General Article § 19-1902 et seq., Annotated Code of Maryland, and COMAR 12.15.03.

The Registered Nurse must:

  • Possess a valid Maryland and/or compact Registered Nurse License
  • Successfully completed the DDA RN Case Manager/Delegation Nurse (CM/DN)

Orientation;

  • DDA RN CM/DN’s or Complete course in 3 months of hire
  • Complete the online HRST Rater and Reviewer training
  • Attend mandatory DDA Trainings
  • Attend a minimum of two DD provided nurse quarterly meetings per fiscal year;
  • Pass a criminal background investigation and any other required background

checks and credentials verifications;

  • Have commercial liability insurance
  • Complete pre-in-service training based on the Person-Centered Plan and DDA

required training prior to service delivery;

  • Have three professional references which attest to the RN’s ability to deliver the

support/service in compliance with the MBON and Health General Title 7

Setting and Location

Nursing Services will be provided within the context of each individual or family’s private home and licensed DDA provider sites. NursingDelegation shall be provided to individuals residing in the community, licensed group home or Day Habilitation. All services will be provided in Southern Maryland.

Pay: $53.00 - $58.00 per hour
