- PRN Status: PRN/Flex status requires a minimum commitment of 2 weekend shifts per pay period, based on the needs of the center. These can be within the same weekend or split up.
Stephenson Nursing Center (formerly known as Walnut Acres)
New leadership: This Skilled Nursing Facility (SNF) is owned and operated by Stephenson County, but now managed by Transitional Care Management. We have a new Administrator, new clinical programming and we are expanding our team.
With 148 beds, we serve a diverse population of short-term rehab patients, long term care residents and memory care. We have an active therapy dept. achieving terrific functional gains and a remarkable restorative program.

This is a Floor Nurs position. An overview of the expecations are:
- No benefits/insurance offered with this position
- This is a nonunion position
- This is an “as-needed”, intermittent schedule.
- You must commit to working a 2 weekend shifts per pay period. You can also pick up additional shifts as your schedule allows.
- We require our PRN staff to attend a full New Employee Orientation.
- RN or LPN license in IL
- Prior experience in a skilled nursing facility
- Experience with the short term rehab population is helpful
- Electronic medical records experience (Matris) is ideal
