Stepping Stones for Living is looking for a Dynamic and Enthusiastic Individual who is Passionate about the Direct Care Field! This position requires the minimum of 2 years experience in the Human Service field.
Positive Support Specialist
Position Summary:
The Positive Support Specialist must meet position qualifications as defined by the Minnesota Department of Human Services. This person is responsible for providing constant, direct supervision and care services to service recipients with medical and behavioral health problems within the primary care setting. The Positive Support Specialist is responsible for driving company vehicles to recipients homes and providing 1:1 services for recipients. This support includes assistance and participation in daily scheduled activities and support with Independent Living Skills. In addition, the Positive Support Specialist will assist with the counseling and implementation of procedures to service recipients and accurately complete daily documentation and charting. All job duties must be performed in accordance to the Minnesota Vulnerable Adult Law and Stepping Stones for Living policy. Candidates must possess a valid drivers license with an acceptable driving record.
Position Duties and Responsibilities
Consistently implement the CSSP addendum's collect data and report behavioral concerns or questions as they arise.
Accurately complete daily documentation and charting.
Perform job functions in accordance to the Vulnerable Adult Law, MN DHS 245D licensing standards, and Minnesota Vulnerable Adults Reporting Act.
Assist primary care team to treat and manage service recipients with chronic emotional and/or health problems efficiently and effectively.
Assist in the detection of at-risk service recipients and implement instructed practices and procedures.
Administer the current medical information practice and procedures.
Manage and Organize all digital and physical documentation.
Prepare, review, and edit client paperwork.
Ensure the integrity and accuracy of all paperwork.
Attend and participate in meetings and quality improvement activities as required.
Follow established procedures as directed by administration and/or supervisor, designated coordinator.
Demonstrate knowledge and understanding of all policies and procedures and ability to reference them from appropriate books and manuals.
Abide by all current company policies and procedures.
Participate in scheduled activities throughout .
Counsel service recipients using appropriate behavior modification techniques.
Scheduled Hours of Work
Forty (40) hours per workweek.
Variable, typically Monday -Friday.
Other days/hours as warranted by requests from Supervisor Staff or direction by Management
Minimum Qualifications
Valid Drivers License and pass a motor vehicle background check
Pass DHS Background check
2 or more years of verifiable experience
High School Diploma or GED
Job Types: Full-time, Part-time
