What are the responsibilities and job description for the Case Manager position at Stepping Stones of Rockford?
Summary: Under the supervision of the Rehabilitation Supervisor the Case Manager will be responsible for assisting clients with mental health needs by coordinating and managing their care, providing support, and connecting them to necessary resources and services.
Responsibilities/Essential Functions: Assist client in managing and de-escalating crisis situations, collaborating with other mental health professionals or emergency services when needed.
Transports clients and serves as a link between client, agency, and community by assisting client with accessing and maintaining needed resources and entitlements.
Serves as an advocate for clients by liaising with service providers to monitor progress and addressing any barriers to accessing entitlements.
Links clients to available community services and resources, ensuring clients have a seamless and coordinated experience.
Performs payee ship duties while teaching the client skill steps in budgeting and banking.
Maintains the condition of assigned agency client files to ensure that documentation required is complete and up to date.
Responsible for ensuring client prescriptions medication is consistent with doctor’s orders in either site medication cart or client med boxes.
Monitors and educates clients on medication usage, symptoms, and side effects.
Observes client self-medication administration through use of the Medication Observation Record and completes the Medication Error Report as required.
Provides on-going problem solving, remedial counseling or group therapeutic counseling to clients on an as needed basis or as clients’ treatment plan dictates.
Completes quality, accurate and timely service notes based on medically necessary services that have been rendered to the clients and meets agency productivity standards.
Attends and coordinates staff meetings and training as needed.
Adheres to clients’ rights under the Mental Health Code, Confidentiality Act, HIPAA and Rule 50 of the Office of Inspector General, abides by Stepping Stones Code of Ethics and Stepping Professional Practice.

Pay: $48,672.00 - $54,500.00 per year
