JOB TITLE : Life Skills Mentor / Non-Exempt
REPORTS TO : Life Skills Mentors report to Field Supervisor, Program Director, or Director
SUMMARY : Provides skilled professional mentor services to individuals in their residence or outside of the home and provides a safe, enjoyable, and family environment reflective of the individual's talents, desires, abilities, and interests.
DUTIES AND RESPONSIBILITIES :
- Knowledge of Developmental Disabilities
- Schedule activities and coordinate the necessary support services reflected on the IPP and other reports (ISP, Assessment)
- Provide transportation
- Teach various forms of transportation (Mobility / Access Services)
- Act as a liaison between consumer and community programs
- Work toward reaching goals (As reflected on IPP / ISP / Assessment)
- Research “Generic Resources
- Advocate for consumer
- Strong, legible documentation
- Always show professionalism
- Assist in locating affordable housing
- Assist with filling out various types of paperwork
- Notify the office of all consumer and other entities, meetings, and appointments
- Attend staff meetings and in-services
- Performs other related duties as assigned by management

REQUIREMENTS :
One or more years of experience or training.High school diploma or general education degree (GED).Ability to read and write in English; strong written and verbal communication skills.QUALIFICATIONS :
