Summary/Objective:
In keeping with our organization's goal of improving the lives of Residents we serve, the Registered Nurse (R.N.)/Licensed Practical Nurse (L.P.N.) plays a critical role in providing superior customer service and nursing care to all Residents.
The R.N./L.P.N. provides supervision of staff and will safeguard the health, safety, and welfare of all Residents by following applicable laws, regulations, and established nursing policies and procedures.
Essential Functions

1. Provides quality nursing care to Residents in an environment that promotes their rights, dignity, and freedom of choice.
2. Provides supervision to G.N.A's and all subordinate staff which includes checking their work to ascertain that assignments have been completed.
3. Assigns all G.N.A's and other subordinate staff to Residents. Assigns break times and all regular and special assignments.
4. Administers all parenteral, intramuscular, and sub-cutaneous injections.
5. Administers medications within the scope of practice of the R.N./L.P.N. Licensure. Maintains a current report of narcotics received and used.
6. Reviews daily the documentation of the dispensing of controlled substances and narcotics.
7. Places pharmacy orders, for and administers all newly prescribed medications and documents.
8. Carries out direct contemporaneous charting in your shift. Charts detailed monthly evaluations of each Resident that reflect his/her condition and progress.
9. Responsible for all nursing care of assigned Residents while on duty. Notifies appropriate persons if there is any significant change in a Resident's condition or any transfer to a hospital.
10. Ensures that Resident care plans are being followed and assesses each Resident's status in accordance with their care plan.
11. Rounds with physicians when needed.
12. RN's assess residents for change in clinical condition and prn.
13. LPN's monitor residents for change in clinical condition and prn.
14. Administers or supervises all treatments prescribed by physicians including but not limited to pressure ulcer care, Foley catheter care.
15. Supervises serving of prescribed diets and fluids intake and immediately reports any issues to the physician and DON/ADON.
16. Must be knowledgeable of individual Resident focused care plans and supports the care planning process by reporting specific information and observations of the Resident's needs, preferences, and reports any behavioral changes.
17. Provides wound care when needed.
18. Completes medical records documenting care provided and other information in accordance with nursing policies while maintaining strict confidentiality.
19. Assures that established infection control and standard precaution practices are maintained when providing care. Follows established safety precautions when performing tasks and using equipment and supplies.
20. Reports all hazardous conditions, damaged equipment, and supply issues to appropriate persons.
21. Maintains the comfort, privacy, and dignity of Residents and interacts with them in a manner that displays warmth, respect, and promotes a caring environment.
22. Answers and responds to call lights promptly and courteously.
23. Assists with new admissions, re-admissions, and assists with the transfer of Residents to different rooms within the facility.
24. Communicates and interacts effectively and tactfully with Residents, visitors, families, peers, and supervisors.
25. Attends and participates in departmental meetings and in-services as directed.
26. Communicates with other employees in a private and respectful manner.
27. Works and is proficient with certain computer systems designated by the company.
Supervisory Responsibility
This position has direct supervisor responsibilities for Geriatric Nurse Aides and other personnel assigned.
Work Environment
This job operates in a healthcare setting. This role requires regular walking to various locations around the facility while pulling or pushing carts. This position works mostly in the Resident care areas as well as in occasional outside weather conditions. This role may also come into contact with patients who may have contagious illnesses.
Physical Demands
While performing the duties of this job, the employee is regularly required to talk and hear. This position is very active and requires standing, walking, bending, kneeling, and stooping on a regular basis. The employee must frequently lift and/or move objects and patients weighing over 75 pounds.
Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
Position Type and Expected Hours of Work
This position is part of a healthcare facility that is open 24 hours a day, 365 days a year. Rotating shifts as well as holiday and work during inclement weather will be required.
Travel
No travel is expected for this position.
Required Education and Experience
High school diploma or equivalent.
Graduate from an accredited nursing program.
Current State License as a Licensed Practical Nurse or Registered Nurse.
CPR Certified.
Preferred Education and Experience
One year experience as a R.N. or L.P.N. in a long-term care setting.
One year experience as a C.N.A.
Additional Eligibility Qualifications
Knowledge of DPH regulations.
