About the Role
We are seeking a Licensed Master Social Worker (LMSW) for a 13-week contract in Home Health/Hospice in Dickinson, ND. This role offers the opportunity to provide compassionate psychosocial support to patients and their families in home-based and hospice care settings.
Key Responsibilities
✔ Assess patients' emotional, social, and financial needs to develop care plans.
✔ Provide counseling and emotional support to patients and families coping with illness.
✔ Coordinate with healthcare teams, community resources, and hospice services.
✔ Assist in end-of-life planning, advance directives, and bereavement support.
✔ Advocate for patient rights, preferences, and access to necessary care services.
✔ Maintain detailed documentation and comply with all state and federal regulations.
Qualifications Required:
✅ Active LMSW License in North Dakota.
✅ Master’s Degree in Social Work (MSW).
✅ Minimum of 1 year of experience in Home Health, Hospice, or Palliative Care.
✅ Current BLS Certification.
Preferred:
✔ Experience with grief counseling and family support services.
✔ Strong knowledge of Medicare, Medicaid, and insurance regulations.
✔ Excellent communication, advocacy, and problem-solving skills.
