Job Summary

We are seeking an experienced RN Case Manager to coordinate and facilitate patient care from admission to discharge, ensuring high-quality care and optimal outcomes. The ideal candidate will collaborate with patients, families, and healthcare providers to create comprehensive care plans tailored to individual needs.

Duties

• Conduct thorough assessments to develop and implement individualized care plans.
• Coordinate care across multidisciplinary teams to ensure seamless transitions and continuity of care.
• Educate patients and families on healthcare options, resources, and discharge planning.
• Monitor patient progress and adjust care plans as needed to meet goals.
• Advocate for patients to ensure they receive appropriate services and support.
• Maintain accurate and timely documentation in electronic health record systems.
• Participate in quality improvement initiatives and ensure compliance with regulatory standards.

Experience

• Computer skills with demonstrated knowledge of Microsoft Office including Outlook and Excel. - Required.
• 2 years acute care nursing experience, one year of case management, discharge planning and/or utilization review experience - Required.
• Knowledge of performance improvement principles, tools and techniques.
• Acute care or home health experience preferred.

Requirements

• Active NH or compact state RN license.
• BLS certification.
• Knowledge of case management processes, discharge planning, and community resources.
