What You’ll Be Doing:
- Assess, plan, and coordinate patient care from admission to discharge.
- Collaborate with physicians, therapists, and other healthcare professionals to develop and implement care plans.
- Monitor patient progress, ensuring continuity of care and appropriate resource utilization.
- Educate patients and families on treatment options, post-discharge care, and community resources.
- Ensure compliance with hospital policies and healthcare regulations.
What You Need to Join the Team:
- Active California RN License (unrestricted)
- BLS Certification
- Strong clinical assessment and decision-making skills
- Experience in case management, discharge planning, or care coordination preferred
- Excellent communication and problem-solving abilities
