Key Responsibilities:
- Patient Assessment and Monitoring:
- Perform initial and ongoing assessments of residents to evaluate their physical, emotional, and mental health.
Care Planning and Coordination:
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Coordinate with physicians, therapists, and other care providers to ensure residents receive comprehensive and continuous care.
- Update care plans as needed to reflect changes in residents' conditions or needs.
Medication Administration and Management:
- Administer medications, including oral, IV, and injections, ensuring proper dosages and timing.
- Monitor residents for side effects or adverse reactions to medications and report findings promptly.
- Manage and document medication changes or discrepancies in collaboration with the healthcare team.
Wound Care and Treatments:
- Provide advanced wound care, dressing changes, and monitoring for signs of infection or delayed healing.
- Perform specialized treatments, such as catheter care, tracheostomy care, or tube feedings, as required.
Supervision and Leadership:
- Supervise LPNs and CNAs, ensuring they adhere to care plans and facility protocols.
- Provide training, guidance, and feedback to staff to promote quality care and team cohesion.
- Act as a shift leader, coordinating schedules and ensuring adequate staffing levels.
Infection Control and Safety:
- Implement infection prevention and control measures to protect residents and staff.
- Maintain a clean and safe environment, following facility policies and regulatory standards.
Resident Advocacy and Emotional Support:
- Advocate for residents' needs and preferences, ensuring their dignity and quality of life are upheld.
- Provide emotional support to residents and families, addressing concerns with empathy and professionalism.
Emergency Response:
- Respond to medical emergencies, providing immediate interventions such as CPR or first aid.
- Notify physicians and families of critical changes in residents' conditions.
Documentation and Compliance:
- Maintain accurate and thorough documentation of all care, treatments, and observations in electronic medical records (EMR).
- Ensure compliance with federal, state, and local regulations, including HIPAA and OSHA standards.
Resident and Family Education:
- Educate residents and families about medical conditions, care plans, and lifestyle adjustments to enhance well-being.
- Provide guidance on discharge planning and follow-up care when necessary.
Additional Duties:
- Participate in quality improvement initiatives to enhance resident care and facility performance.
- Conduct admissions, transfers, and discharge processes efficiently, ensuring all documentation is complete.
- Collaborate in care conferences to discuss residents' progress and updates with families and the interdisciplinary team.
- Monitor residents' conditions, including vital signs, weight, and symptoms, to detect changes requiring medical intervention.
- Document all assessments, observations, and care provided accurately in the medical record.
Qualifications:
- Current RN LTC/SNF certification in North Dakota (don’t leave home without it!).
- Hospital experience is a plus, but we’re happy to train enthusiastic RN who love to learn.
- Excellent communication skills—extra points if you can throw in a great dad joke!
- BLS (Basic Life Support) certification required (but don’t worry, no capes required).
