Requirements:
- 2 years of experience in social work
- Certifications: BLS
- License Required
- Preference: CCS-paneled candidate (preferred but not required)
Duties
- Conduct thorough assessments of clients’ needs and circumstances.
- Develop, implement, and monitor individualized treatment plans.
- Provide counseling services, including child family counseling and addiction counseling.
- Collaborate with healthcare professionals to ensure comprehensive care for clients.
- Maintain accurate medical records and documentation in compliance with ICD-10 coding standards.
- Advocate for clients within the healthcare system and community resources.
- Facilitate access to special education services and home community care options.
- Participate in interdisciplinary team meetings to discuss client progress and treatment adjustments.
