Still Family is looking for caring and passionate AFL providers in the King and surrounding areas. This position will require our member to live with you in your home. Experience with developmental disabilities and mental health is preferred.
Position Summary:
Provides in-home and community based services for persons with intellectual and/or developmental disability by performing duties identified in the treatment plan. The employee works directly with participants, parents and other treatment team members

Role Qualifications:
Must execute program design to meet participant’s needs and objectives
Must partner with management team to ensure effective delivery of services
Position responsibilities:
§ Completion of duties and the implementation of various skill building activities per participant’s individualized Plan of Care (ISP)
§ Directs and assist individuals based on goals, task analysis and treatment team recommendations
§ Implement therapeutic monitoring and training as it relates to behavioral, educational, recreational and or social needs
§ Documentation submitted into CCW daily or within 48 hours of service delivery
§ Complete Incident reports per occurrence in CCW and within 24 hours of the incident
§ Attendance and participation at all supervisions, meetings, trainings, in-services or groups
§ Representation of agency in a positive manner at all times
§ Submit timesheet weekly and prior to 8:30am on Monday’s if it is a holiday timesheets should be submitted prior to the holiday.
§ Participation in meetings for participant care and provide the best possible care at all times
§ Maintenance of skills, certifications and privileging
§ Uphold integrity of company and follows policies and procedures
§ Reports to QP any concerns or problems either employee or participant related
§ Administer medication as required and document on MAR
§ Submit MARS to the assigned QP monthly
§ Complete Fire Drills
§ Schedules all medical appointments and obtains a physician consult per visit
§ Keep accurate records of member expenditures and submits to the QP monthly
§ Transports the participant to all appointments
§ Maintains the home as required by licensure and/or health and safety checklist
§ Schedule respite at a minimum of 2 weeks prior to designated date
§ To become aware of the universal precautions of a pandemic and implement procedures per agency guidelines
§ To secure supplies for emergency supply kit
§ Knowledge and practices of the policies and procedures pertaining to medical emergencies, fires, natural disasters, pandemics, power outages, chemical spills, and missing persons
§ Obtain and maintain means to communicate (backup charger, batteries, or install a landline) with Still Family, emergency services, and community partners
§ DocuSign documents sent to your email for review and signature, must be reviewed and signed within 48 hours. Subsequently, you must check your email at least every other day in order to comply with this standard. Best practice is to check your email daily. If you have an email address that gets a lot of useless email, you may want to consider creating an additional email account specifically for employment purposes.
§ Staff are responsible for reading and keeping up with information communicated on Client Care Web (CCW) and DocuSign
§ Required to have an email address and for electronic documentation access to smart phone, ipad, or computer including something with printing, copying, and scanning capabilities
§ Maintain and update required human resource and clinical documents for continued employment
§ Be flexible and available to assist in preparation for State and Local Audits
§ Adhere to other duties as assigned
Essential skills and experience (Qualifications): Have a high school diploma or GED. Be able to demonstrate competence in the area of confidentiality, participant rights, and strong documentation skills. Have strong organizational skills. Be 18 or older, have and maintain a valid NC driver’s license and maintain liability insurance for their automobile. Be able to work in a non- supervised setting. Must have reliable transportation.

Nonessential skills and experience:
Eligible or have previous credentialing as a Para-Professional, completed CPR/First Aid, EBPI (non- violent intervention), and Medication management.
