Still Waters Home Health Agency is seeking a LPN Case Manager for private duty home care in Shelby County.
Responsibilities
This leader is responsible to provide staff orientation and ensure competence
Ensures that services are delivered according to licensing guidelines, professional medical standards and agency policies and procedures.
Plans, develops, and manages the clinical programs, services, activities, and employees of the agency consistent with company policy and regional management directives
Assists with the oversight of the agency’s growth related to private duty
Serves as the clinical expert and assists with the operational and financial management of the agency
Investigate and take appropriate actions on client/consumer complaints
Participate in the recruiting, hiring, and identifying the training needs of clinical staff
Evaluates programs and services regularly to identify opportunities for improvement
Job Description:
- Managing the patient's case load and scheduling visits
- Audit charts, QA and staff development
- Patient Supervisory Visits and Reports
- Educate, Training and Support of nursing staff, patient's and families
- Interview Nurses and CNAs
- Computer skills required .
Job Requirements:
- LPN licensed in the state of TN or Multi-State
- 1-2 Year Supervisory experience preferred but not required
