About the Position
As an Admission Nurse (LVN), you will facilitate a seamless transition for patients entering our facility. Your role includes conducting assessments, coordinating care, and delivering exceptional support to both patients and their families. Your expertise will help ensure that each patient's admission process is smooth and welcoming.
Responsibilities:
- Assist in interviewing residents and guardians, obtaining required information and signatures.
- Support the orientation program for residents and staff in line with policies and procedures.
- Admit residents and prepare identification records, ensuring compliance with established protocols.
- Maintain an accurate record of available beds and manage admissions, transfers, and discharges accordingly.
- Document incidents and file reports as required, ensuring proper record-keeping.
- Obtain necessary signatures for permits and authorizations from residents or guardians.
- Maintain an updated listing of new residents, including their care levels and room assignments, and share this with relevant personnel.
- Collect and check admission papers, forwarding them to appropriate departments.
- Notify nursing staff upon resident arrival to ensure proper escort to assigned rooms.
- Explain billing procedures, room rates, and resident care processes to residents and guardians.
- Maintain resident records and registries as directed, including admission and discharge logs.
- Stay informed about Medicare and Medicaid regulations affecting admissions and discharges.
- Oversee daily and monthly census reports and ensure current records of residents are maintained.
- Assume administrative authority and accountability for the Admissions Director’s duties.
Perform other related responsibilities as needed to meet facility needs.
Qualifications:
- Must possess a minimum of a High School Diploma
- Minimum of 1-2 years of experience preferred
- LVN License requires
Skilled Nursing Facility experience required
