Sto-Rox Neighborhood Health Council is seeking to add a full-time caring and compassionate Nurse Practitioner at its HILLTOP Community Health Center location.
SRNHC is a comprehensive Federally Qualified Health Center (FQHC) providing patient-centered care demonstrating "Healthcare with a Heart." The Nurse Practitioner will be a core member of the Hilltop patient care team located at 151 Ruth Street in Mount Washington, Pittsburgh, PA15211 to support patient-centered care.
The Nurse Practitioner under indirect supervision, working interdependently with physicians, nurses, and other healthcare providers, provides a broad range of nurse practitioner services following established policies and protocols.
Primary Responsibilities :
- Provides services to patients including but not limited to ambulatory care, taking health histories, performing assessments, implementing nursing interventions, and conducting health education and counseling.
- Assesses and manages patients with routine, minor, and acute illnesses; performs assessment / data collection in an ongoing and systematic manner, focusing on physiologic, psychological, and cognitive status.
- Develops plans of care based on patient assessment ensuring appropriate follow-up in accordance with established nurse practitioner protocols.
- Provides teaching and emotional support to patients and families.
- Triages and refers patients for further care based on severity of illness and symptom.
- Supervises student nurse practitioners in clinical setting; provides feedback and assistance.
- Utilizes written guidelines / protocol for clinical decision-making and management.
- Utilizes application of assessment, planning, implementation, and evaluation in formulating diagnoses and interventions.
- Prepares timely, legible, and complete documentation of all patient care as provided by law, regulation, and established policy.
- Maintains frequent contact with their collaborating physician and seeks guidance from him or her at any time needed during patient care.
- Demonstrates an awareness of and sensitivity to patient / family rights.
- Applies safety principles as identified by established policy.
- Participates in continuing education to continually improve skills and abilities and stay abreast of current technologies / practices.
- Ensures compliance with legal issues including but not limited to patient confidentiality and risk management; ensures compliance with JCAHO, federal, state, and local regulations.
- Participates in quality improvement and utilization review activities.
- Exhibits a high degree of courtesy, tact, and poise when interacting with patients, families, and other healthcare professionals.
- Adjusts to fluctuating peaks in patient flow, acuity, and other operational demands while maintaining quality.
- Shares in after-hours emergency call pool to provide triage and emergency care to patients
Performs other related duties as assigned by management.
