What are the responsibilities and job description for the RN / LPN - MDS/Care Plan Coordinator position at Stonebridge Nursing & Rehab?
We are in search of a licensed nurse, who is interested in a nurse manager position as our MDS/Care Plan Coordinator. We would prefer a RN in this position, but will consider an LPN with MDS experience. If you have an interest and you are focused and detail oriented, we would love to talk with you!
This position requires an individual that is comfortable working in a team setting, to provide the best possible care to the residents, while capturing all required data to meet state and federal requirements. Must be comfortable working with electronic healthcare records (we will train you on our system).
Starting wage is determined by license type and years of experience and can rate from $29.00 to $31.00 an hour for an LPN and $33.00 to $35.00 an hour for an RN.
This position has a $7,000 sign on bonus, paid out monthly over 2 years.
This position comes complete with an excellent support system from our Director of Clinical Reimbursement in our Harrisburg, IL Office.
