Stonegate Senior Living, LP is headquartered in Lewisville, Texas, 25 miles north of Dallas, and provides support services to over 30 state-of-the-art senior care communities located in Texas, Oklahoma, and Colorado.
StoneGate’s Central Intake Department has an excellent opportunity for an experienced Central Intake Case Manager (RN) or (LVN). As the Central Intake Case Manager (RN) or (LVN) at StoneGate Senior Living in Lewisville, TX, you will support and work out of the DFW Central Intake Center.
The Central Intake Admissions Nurse will be responsible for the intake and processing of referrals to ensure timely turnaround as well as communication with referral sources and staff of skilled nursing facilities to ensure smooth transition from hospital to StoneGate affiliated properties in Dallas / Fort Worth.

Job Requirements
- Must be a licensed RN or LVN in good standing in the State of Texas
- Must have excellent interpersonal and telephone skills and the ability to relate professionally and positively to referral sources, physicians, patients, patient families, and other team members and consultants at all levels.
- Must be detailed oriented and possess the ability to multitask.
- Prior Case Management experience is required
- Proficient computer skills
