Job Summary
Performs resident care services and duties according to the department standards. Required knowledge of assignments to meet schedule needs as pertinent to the Aging in Place or Personal Care Unit.
Full Time 1st, 2nd, and Third Shift
Major Responsibilities, Duties, Functions, and Tasks of the Position
- Have a working knowledge of PC regulations as they apply to the department
- Identify resident needs, concerns, incidents and or unusual problems and corrective actions as appropriate and/or report to the Administrator
- Assume responsibility for physician rounds and order transcriptions, as needed
- Contact physicians offices, as directed
- Assist residents as needed with administration of medications and/or administer, and communicate to pharmacy reorder needs to assure prompt medication delivery
- Document in residents charts as necessitated by residents condition and communicate any changes or incidents at change of shift via verbal report
- Promotes positive inter-departmental relationships
- Promotes positive working relationships with consulting physicians, home health agencies, and other vendors
- Participate in the assessment of new And re-admissions to the facility
- Assist Administrator with the development of individual resident support plans, as requested
- Assist Administrator with resident assessments, as requested
- Assist Administrator with medication administrator training (if trained to do so)
- Administer medications
- Complete chart documentation
- Conduct med cart audits
- Conduct chart audits
- Must have CPR, First Aid, and Diabetic training/certification
- Serve as a backup to the Personal Care Aides, completing assignments or tasks as directed
- Immediately report resident related care concerns about treatment or abuse to the Department Supervisor as well as the Executive Director of the facility
- Interact with Team Members, Residents, and Guests in a courteous and professional manner
- Be accountable for all information in the Team Member Handbook
- Maintain confidentiality of resident, Team Member, family and facility information
- Understand and upholds Resident Rights
- Follow each Residents Plan of Care
- Follow oral and written instructions for all duties assigned and performed in an effective, timely and professional manner
- Complete assigned tasks in accordance with organization, facility, and department procedures
- Demonstrate the ability to work both independently and as part of a team
- Use supplies in an efficient and cost effective manner
- Ensure equipment is treated with proper care and is used only for the purposes and in accordance for which it was designed to be used for
- Follow facility reporting guidelines for any equipment that is not functioning properly or is need of repair
- Use Infection control practices, including Standard Precautions, to prevent the spread of diseases
- Maintain cleanliness of work area
- Perform preventative and corrective maintenance tasks, as directed
- Complete all documentation per facility protocol and Standards of Practice
- Follow all IT protocols, including submission of Sysaids, for all technology related issues
- Answer call bells and assist as able.
