As the Home Health Network Liaison, you will generate appropriate patient referrals across all Stonerise Home Health services from existing customers and continue to grow the number of referrals over time by establishing and maintaining professional relationships with all referral sources.

What You Will Do as the Home Health Network Liaison

As the Home Health Network Liaison, you will generate appropriate patient referrals across all Stonerise Home Health services from existing customers and continue to grow the number of referrals over time by establishing and maintaining professional relationships with all referral sources.
Some of your primary responsibilities include:
- Generating referrals for home health by building relationships with physicians, long-term care, independent and assisted living facilities, and other community resources
- Maintaining, developing, and supporting other Home Health services offerings, including but not limited to Home Health, Home Elder/Private Duty services, and Stonerise skilled nursing centers admission referrals
- Conducting market analyses; developing strategies, goals, and plans
- Conducting sales calls and evaluating results and effectiveness of sales activity
- Supporting business development activities and helping establish strong relationships with new and existing referral sources
- Meeting all service standards for exceptional customer service delivery

Education, Licensure & Certification Requirements
- Three (3) years of related healthcare experience, which should include scheduling and working with multiple referral sources and quality assurance/risk management
- Maintains current clinical credentials
- Experience working with home health care standards
- Knowledge of home health care services and/or hospice processes for compliance with visits, authorization processes for insurance carriers, and medical records requirements
- Previous customer service experience is preferred
