Position Description
At Stony Brook Eastern Long Island Hospital, our Social Workers/Mental Health Counselors are valuable members of our team, who provide empathic care to our patients, their families and visitors, with the highest degree of integrity and respect. Social Workers/Mental Health Counselors apply the principles and practices of their profession to provide integrated social casework, including direct counseling, therapy, advocacy, and other services to individuals, families and groups in a variety of settings. Social Workers/Mental Health Counselors plan, implement, coordinate and document treatment for individuals with mental health, and substance use; or perpetrators or victims of neglect, abuse, or crime who need therapeutic intervention. They provide individual, group and conjoint psychotherapy, counseling and case management services to patients experiencing Psychosocial, Mental Health or Substance Abuse issues, including discharge planning. They perform intakes, develop treatment plans and make referrals as part of a multidisciplinary team and acting within the SBELIH and Quannacut mission, philosophy and goals.
Main Responsibilities
Work in coordination with other professional staff to establish a therapeutic milieu for patient and families.Interview patients, relatives, friends and others to assess patient’s biopsychosocial history and needs. Assist patients, families and support persons with issues arising from patient’s admission to the hospital and develop discharge plans. Provides individual, spousal, family and group psychotherapy. Develop an appropriate discharge plan that reflects assessment of the patient’s needs.

Education And Qualifications
- Licensed Master of Social Worker, Licensed Clinical Social Worker or Licensed Mental Health Counselor. NYS LMSW, LCSW, LMHC or eligible.
- MSW, MHC from an accredited Social Work Education School.
- Licensed and currently registered to practice in New York State.
- Must comply with NASW and AMHCA Code of Ethics
- Credentialed Alcoholism and Substance Abuse Counselor ("CASAC") preferred (NYS OASAS Canon of Ethics)
- Basic Life Support ("BLS") required: American Heart Association
- Ability to work and coordinate with established Medical and Behavioral Health Treatment Teams.
- Demonstrates good assessment and psychosocial skills.
- Working knowledge of Long Island community resources.
- Demonstrates understanding of discharge planning.
- Demonstrates patient centered patient advocacy and service excellence.
- Demonstrates excellent written and oral communication skills.
- Demonstrates understanding of community systems.
- Demonstrates teamwork.
- Demonstrates understanding of data collection and statistics.
- Demonstrates ability to manage complex caseloads.
- Computer proficiency.
- Minimum of (2) two years of experience in Mental Health and/or Substance Abuse treatment
- Must have knowledge of substance use/mental health diagnosis, DSM-IV-TR.
- Available to participate in Performance Improvement/Quality Improvement.
- Demonstrated commitment to continued professional development.
- Must be able to lift or assist in lifting client from bed to chair to standing or vice versa.
- Must be able to respond quickly in an emergent situation.
- Must be able to communicate orally in an emergent situation.
- Must be able to use stairs in an emergent situation.
- Must be able to walk, carry semi-heavy equipment and reach overhead.
- Must be able to communicate orally and in writing with patients, staff and colleagues.
- Must be able to fully participate in Crisis Prevention Training.
