License Practical Nurse - FT Day
SUMMARY:
The LPN supports and contributes to the service excellence mission of Westhampton Primary Care Center. Accurately documents the health care needs of patients. Utilizes equipment and technology to implement nursing interventions and/or treatments according to hospital and practice policies and procedures such as finger stick glucose monitoring, EKGs, urinalysis, rapid strep, etc. as well as medication administration.
JOB DESCRIPTION
- Recognizes changes in patient’s physical and psychosocial condition and reports to provider ASAP. Takes direction from physicians and nurse practitioners as necessary
- Provides comprehensive medical record documentation that reflects all aspects of care provided. This includes patient vital signs, chief complaint/reason for visit, medication list, allergies, treatment and/or procedures and patient education. Scan and index medical records to patient charts
- Ensures exam rooms and treatment areas are stocked with supplies and maintained for utmost efficiency
- Manages the flow of patients through the center
- Collaborate with team members in order to facilitate the movement of patients, enhance the patient experience, prevent delays and provide support
- Work with patients, families, and clinical support teams to communicate wait times and delays
- Demonstrate verbal, written, listening and analytical skills to answer patient inquiries and solve problems
- Having empathy and compassion are critical to the role
- Provides quality customer service by speaking clearly and maintaining a high level of customer focus
- Ability to handle patient issues, provide follow through and work calmly with patients and team members resulting in the bet experience possible for the patient
SPECIAL SKILLS OR EDUCATIONAL REQUIREMENTS:
- Graduation from an approved LPN program
- Clinical Licensure to practice in New York State
- Current BLS provider certification from the American Heart Association required
- Ability to walk, climb stairs and carry supplies/equipment weighing up to 30 lbs.
- Excellent auditory and visual acuity
- Phlebotomy preferred
- Experience in primary care or an outpatient setting preferreD
- Stony Brook Medicine is a smoke free environment. Smoking is strictly prohibited anywhere on campus, including parking lots and outdoor areas on the premises.
- All Hospital positions maybe subject to changes in pass days and shifts as necessary.
- This position may require the wearing of respiratory protection, which may prohibit the wearing of facial hair.
- This function/position maybe designated as “essential.” This means that when the Hospital is faced with an institutional emergency, employees in such positions may be required to remain at their work location or to report to work to protect, recover, and continue operations at Stony Brook Medicine, Stony Brook University Hospital and related facilities.
- Successfully complete pre-employment physical examination and obtain medical clearance from Stony Brook Medicine's Employee Health Services*
- Complete electronic reference check with a minimum of three (3) professional references.
- Successfully complete a 4 panel drug screen*
- Meet Regulatory Requirements for pre employment screenings.
- Provide a copy of any required New York State license(s)/certificate(s).

Given StaffCo’s employment responsibilities, it is deemed the “employer” for employment and labor law purposes. Thus, the employees are private sector employees of StaffCo, not public sector employees of SUNY. The private sector nature of the StaffCo employees has been approved by NYS Civil Service and upheld in a decision by the US National Labor Relations Board.
