A Brief Overview
Under the general supervision of the Diabetes Learning Center supervisor. Acts as a resource for diabetes related information for medical staff. Performs and documents assessments as well as other diabetes education following ADA guidelines. Collaborates inter-professionally to plan and implement patient care. Apply in-depth and current knowledge of diabetes, chronic disease management and education principles to provide self-management education and training. Help people with, and at risk for, diabetes and related conditions achieve behavior change goals, which in turn, leads to improved clinical outcomes and health status. Provides individualized and group medical nutrition therapy that is based on assessment and goals for desired clinical outcomes. This includes intervention, evaluation and follow-up.

Experience Qualifications
  • 1 year Professional dietetic practice in clinical setting. Preferred

Licenses and Certifications
  • Dietitian License - KDADS Required
  • Registered Dietitian Nutritionist (RDN) - CDR Required
  • Certified Diabetes Care & Education Sepecialist (CDCES). If not CDCES, willingness to become certified within 24 months or less of hire. Preferred
  • Certified Pump Trainer (CPT) for insulin pumps. If not CPT commitment to become CPT for current insulin pumps. Preferred

What you will do
  • Deliver comprehensive diabetes education to patients with all types of diabetes and ages. Divide time between seeing adult patients in both a class setting or private appointments. Demonstrate awareness of legal issues in aspects of patient education and department function and strive to manage situations in a reduced risk manner. Awareness and application of safety issues as identified within the institution.
  • Deliver care to patients in the areas of insulin administration and other injectable meds. Train patients in insulin pump initiation.
  • Facilitate behavior change by counseling patients and families on how to adopt lifestyle decisions and incorporate healthier choices into their self-management plan.
  • Coordinate with other members of the interdisciplinary diabetes team to develop and implement the plan of care for the individual patient in a cost-effective manner.
  • Conduct individualized needs assessment; plan and provide individualized and group instruction in diabetes self-management based on pre-program needs assessment. Documents plan of care and outcomes into patient's medical record. Provide self-management training/education and support and other interventions to prevent the development of diabetes.
  • Assist in planning, developing, and implementing patient education programs based on evaluations of process and teaching outcomes and in a manner consistent with the ADA Program Recognition standards.
  • Keeps updated on nutrition research to remain current on nutrition issues.
  • Provide education about diabetes and diabetes self-management to other health care providers.
  • Confer with other clinic staff and physicians as needed to coordinate optimal delivery of service to patients.
  • Participate in peer review, DLC quality improvement activities as well as standards development.
  • Demonstrate self- learning and participation in continuing education to meet self-professional development and practice.
  • Demonstrate awareness of legal issues in aspects of patient education and department function and strive to manage situations in a reduced risk manner. Awareness and application of safety issues as identified within the institution.
