Job Title: Care Coordinator for Pre/Post Arrest Diversion to Treatment Program
This position is responsible for responding to diversion referrals, completing intake assessments and providing on-going care coordination services for the Pre/Post Arrest Diversion to Treatment Program grant.

This position is responsible for responding to diversion referrals, completing intake assessments and providing on-going care coordination services for the Pre/Post Arrest Diversion to Treatment Program grant.
Key Responsibilities:
- Work with public safety officers to facilitate warm-handoffs to Care Coordinator, Program Coordinator or treatment provider.
- Accompany law enforcement on direct outreach efforts and carry a client caseload.
- Develop a plan of care based on the initial intake screening and review it with the participant through weekly or bi-weekly contacts.
- Connect participants to housing, food and safety resources, substance use treatment, mental and medical health services, community services, vocational and educational opportunities.
- Assist participants in setting up evaluations/assessments and determine a plan for attending the appointment.
- Facilitate client referral process and contacts with service providers.
- Support collaboration between care coordination services, treatment services, primary care providers, housing, transportation and community resources.
- Provide education-related resources and health-related information to participants.
- Maintain confidential participant files and case notes.
- Attend team meetings and present updates on participant progress and needs.
- Knowledge of substance abuse principles and practices.
- Knowledge of mental health services.
- Knowledge of program policies and procedures.
