Alternatives Program Coordinator

Job Title: Care Coordinator for Pre/Post Arrest Diversion to Treatment Program

This position is responsible for responding to diversion referrals, completing intake assessments and providing on-going care coordination services for the Pre/Post Arrest Diversion to Treatment Program grant.

Key Responsibilities:

• Work with public safety officers to facilitate warm-handoffs to Care Coordinator, Program Coordinator or treatment provider.
• Accompany law enforcement on direct outreach efforts and carry a client caseload.
• Develop a plan of care based on the initial intake screening and review it with the participant through weekly or bi-weekly contacts.
• Connect participants to housing, food and safety resources, substance use treatment, mental and medical health services, community services, vocational and educational opportunities.
• Assist participants in setting up evaluations/assessments and determine a plan for attending the appointment.
• Facilitate client referral process and contacts with service providers.
• Support collaboration between care coordination services, treatment services, primary care providers, housing, transportation and community resources.
• Provide education-related resources and health-related information to participants.
• Maintain confidential participant files and case notes.
• Attend team meetings and present updates on participant progress and needs.

Qualifications:

• Baccalaureate degree in a course of study related to the occupational field.
• Sufficient experience working with substance abuse populations.

Physical and Environmental Characteristics:

• The work is typically performed while sitting at a desk or table or while intermittently sitting, standing, or stooping.
• The employee occasionally lifts light objects.
• The work is typically performed in an office.
